The Consolidated Contract (ConCon) web pages are designed for use by DOH and 35 Local Health Jurisdictions (LHJs) that contractually receive state and federal funds from various programs within the state Department of Health for local public health services. 

The purpose of this ConCon website is to give LHJs electronic access to the most current information about the Consolidated Contract.  It also enables electronic submission of completed contract materials via the Con Con mailbox. 

You are also encouraged to use the ConCon mailbox for sending questions and comments related to the contract. All correspondence and contract materials sent to the DOH ConCon mailbox will be distributed to the appropriate DOH programs/staff for response.

The following gives you information about the specific ConCon web pages:  

• LHJ Menu links to a listing of all 35 LHJs.  Click on your LHJ to access to your current contract materials.  

• Updates has recent announcements 

• Contact Lists contains lists of DOH Contracts Staff, DOH ConCon Program Staff, and LHJ ConCon Coordinators.  

• Forms, Reports and Program Information lists program-specific information and/or  required forms and reports.

• Billing Instructions contains billing instructions for the 2007-2011 contract.
