Laboratory Information

Reporting

WAC 246-101 requires laboratories to report all blood lead test results to the Washington State Department of Health. All elevated blood lead levels (10µg/dL or higher in youths under age 15, and 25µg/dL or higher in adults) must be reported within two days.  All other test results must be reported within one month.


Information provided must include:

  • specimen type

  • name and telephone number of laboratory

  • date specimen collected

  • date specimen received

  • requesting health care provider’s name and telephone number or address

  • test result

  • name of patient (if available) or patient identifier

  • sex

  • date of birth or age of patient (if available)

  • patient’s address when available, in order to facilitate public health follow-up of cases with elevated blood lead levels.

 


Blood lead test results may be faxed to 360-236-3059 or mailed to:  Laboratories can download, but are not required to use the Department of Health's Blood Lead Test Report Form.
