Reciprocal certification is provided only to individuals who have successfully completed the Oregon State sponsored Basic Decontamination Worker Course and submitted the completed Washington State Department of Health's CDL Decontamination Worker application to DOH within 60 days of completing Oregon's Worker Course.

 
 Step 1

Read:

CDL Worker Certification application form pdf  40 kb in detail to ensure that you understand the requirements for certification.

 

 Step 2 Complete:

Forty-hour Hazardous Waste Operations (HAZWOPER) training must be successfully completed in order to attend the DOH 2 day CDL Worker Certification Course.  HAZWOPER classes are offered through private companies and community colleges. Search the Internet for HAZWOPER training in your area.

Your HAZWOPER certification must be current in order to attend the CDL Worker Certification Course.

     Worker Certification lasts for 2 years.
