Report an Injury

Pool operators of regulated facilities must report any death, near drowning, or serious injury. Complete the injury report form and contact your local health department within 48 hours of the incident. If you can't reach them, contact us.

Report an Illness

Contact your health care provider if you think you have a recreational water illness. Report suspected recreational water illnesses to your local health department.
