2. Credit card number
3. Credit card expiration date
4. Address certificate is to be sent to. If you are requesting UPS Next Day Air delivery (for an additional fee) we must have a street address. UPS Next Day Air cannot deliver to a post office box.

Fees for each certificate:

• $32.00 US (Pay by credit card)

For Expedited Delivery you must add

• Express Mail, and additional $16.50 US
• UPS Next Day Air, an additional $20.00 US
• UPS AK, HI & PR, an additional $25.50 US
• UPS Canada/Mexico, an additional $26.50 US
• UPS International, an additional $37.00 US
