Ordering by mail normally takes 3 to 4 weeks to receive a certified copy of a certificate.

Fees for Each Certificate:

• $20.00 US - (Make checks/money orders payable to Department of Health)

For Expedited Delivery you must add

• Express Mail, an additional $17.50 US
