The service counter is open to the public 8:00am - 4:30pm Monday through Friday, except state holidays. For further information please call (360) 236-4300.

You can download and fill out the ordering form, and bring it to counter at the address above.  Alternately, the counter has the same forms that you can fill out in person.

Birth and death certificates can also be obtained from your local health department.  Many local health departments have same day service, while others have next day service.

Fees

Fees for each certificate ordered:

• $20.00 US - (Make checks/money orders payable to Department of Health),
  Walk-In Same Day Service does NOT accept credit cards.
