Comprehensive Hospital Abstract Reporting System (CHARS) 1987 to 2011Q2

The Comprehensive Hospital Abstract Reporting System (CHARS) has coded hospital inpatient discharge information (derived from billing systems) available for 1987 to 2011Q2. Coded hospital-based observation data is available from 2008 forward. CHARS is used to collect information such as the age, sex, zip code and billed charges of  patients, as well as the codes for their diagnoses and procedures among other items. The purpose of the CHARS system is to provide public health personnel, consumers, purchasers, payers, providers, and researchers with information to use to make informed decisions on health care. The CHARS system provides those concerned with the development of public policy with information necessary to analyze many significant health care issues. Specifically, the Department of Health uses the CHARS data system to:

  • Identify and analyze health trends related to hospitalizations;
  • establish statewide diagnosis related groups (DRG) weights, a way of comparing hospital stays across all hospitals;
  • create hospital specific case mix indices, a way to compare the types of stays at hospitals; and
  • identify and quantify issues related to health care access, quality, and cost containment.

Reports of aggregated discharge counts, patient days, payers, patient origin as well as footnotes on the data.

CHARS UB-04 Information
CHARS collects the data provided on the Medicare UB-04 billing form.  This page has information on the how DOH requires that data to be submitted, including information about the 837I 4010 file format currently used by CHARS.

CHARS 837I 5010 Transition
The hospital industry is converting its billing information to the 837I 5010 file format.  Because CHARS data is collected using industry-standard files, it will also convert to that standard.  This page has information on the department's transition process to collect CHARS data in the 5010 file format.

Use this form to order the public data set. The form may be used to order one or several years' data for download.  Access to the confidential data set is strictly limited by state privacy regulations.

File Layout Information
CHARS Public File Layout Information
This file layout tables list the de-identified or public data elements available in the CHARS Public Dataset and includes some code dictionaries.
There are five tabs in the file, one for each table in the inpatient dataset.
Clinical,
which contains demographic and clinical information (including the first 9 diagnoses, first 6 procedures and 1st ECode (external cause of injury)) for each discharge
Revenue,
which contains the revenue codes collected by CHARS for each discharge
Diagnosis, which contains all the diagnosis codes for each discharge
Procedures, which contains all the procedure codes for each discharge
ECode, which has all the ECodes for each discharge

Hospital Directory for CHARS Hospitals
This file lists the License Number, Hospital Name, CHARS Unit Type, Address, City, State, Zip Code, County for the community hospitals in Washington.

CHARS History
A chronological listing of the major changes in hospital status over the years CHARS data has been collected. This includes name changes, DOH license number changes, Medicare license number changes and other pertinent information.
