Adverse Event Reports

We generate adverse events reports on a quarterly basis and represent aggregate information. We post reports within 90 days after the reporting quarter.

We do not display specific details about events, such as patient and staff identification, outcome of event, etc., on these reports. However, facilities reporting an event can voluntarily provide descriptions or other more information on the contextual information form.

Adverse Events Table (58KB PDF)

This is a summary report displaying the total number of events reported by event type and reporting quarter. The report reflects reporting from July 2006 through the most recent quarter.

Facility reporting Events by Quarter (96KB PDF)

This report lists all healthcare facilities by type and the number of adverse events reported by each of these facilities every quarter since July 2006. We have included responses to the check-in survey in this report.

Facility reporting by Event Type (104KB PDF)

This report displays facilities and the type and occurrences for each adverse event type.

Facility by Date (187KB PDF)

This report lists the facility name, confirmation date of reported adverse event, and event type. This report has been re-formatted and is a chronological listing of events by facility type and facility name.

Contextual Information Reports

Healthcare facilities send these reports to provide more information on a reported adverse event. Select the year of the event to locate a specific Contextual Information Report:

Root cause analysis reports and its contents are confidential. We will not release these to the public.
