Who needs a license?
All ambulatory surgical
facilities in Washington State need a license. An ambulatory surgical
facility is any facility that performs outpatient surgeries and uses
general anesthesia. Such facilities are described in state law. (RCW
70.230.010(1)) and
RCW
70.230.040(3)).

Any facility or office
that does not use general anesthesia does not need, and cannot get, this
license. In addition, hospitals and dental offices are not required to
have this license.
The facility should submit
a written request stating it does not use general anesthesia and wishes
to have the license fee refunded. This letter needs signed by the
requestor and sent via US mail to the Department of Health. Mail this
letter to DOH, PO Box 47877, Olympia, WA 98504. The department will
refund any license application fees received from any such facility or
office.
If you use medications in
your facility and need a DEA registration, you need to hold a Health
Care Entity-Pharmacy license. If a facility paid a fee for an ASF
license in lieu of renewing the entity license, we will adjust the
payment submitted for the ASF license to cover the $265.00
renewal fee for the entity license. The remainder
of the ASF license payment will be refunded.
