Licensing Requirements

All ambulatory surgical facilities (ASF) in Washington State need to be licensed. The state laws relating to ambulatory surgical facilities can be found in chapter 70.230 RCW. . For a description of ambulatory surgical facilities, visit FAQs

New Ambulatory Surgical Facilities must complete and submit an application and Fee, including:

  • Name of managing personnel, officers, and administrator.
  • Description of the organizational structure.
  • Name, address and phone numbers of all office locations.
  • Copy of current business license.
  • Proof of completion of the department’s construction review process.
  • Copy of safety & emergency training program.
  • Proof that a certificate of occupancy by the local building official has been approved and issued.

Frequently Asked Questions

If I do not qualify for an ASF license, do I need to be licensed as a health care entity?

If you use medications in your facility and need a DEA registration, you must hold an active health care entity-pharmacy license. If a facility paid for an ASF license in lieu of renewing the health care entity license, we will adjust the payment submitted for the ASF license to cover the renewal fee for the health care entity license. We will refund the remainder of the ASF license payment.

How do I switch from the ASF license to a pharmacy health care entity license?

For those who qualify, to cancel your ASF license and apply for a health care entity license, you can download the application from health care entity application packet. Send us your application and include a letter explaining that you wish to cancel your ASF license and become licensed as a health care entity.
