CN11-35 - Evergreen Hospital Medical Center  (PDF, 2,035KB) - Proposal to add 6 NICU level III bassinettes to the hospital's neonatal program.

CN12-07 - Valley Regional Hospital, LLC  (PDF, 1,966KB) - Proposed lease agreement between Snohomish County Public Hospital Dist. #1 and CMCH Holdings to allow Capella Healthcare, Inc to manage the Valley Regional Hospital's operations.  (Certificate)

CN11-03 - Gateway ASC, LLC  (PDF, 2,070KB) - Proposing to convert an existing ASC in Snohomish County into a CN approved facility.

CN11-16 - Eastside Endoscopy Center, LLC  (PDF, 1,145KB) - Proposal to establish an endoscopy ASC in Issaquah, within east King County.
