Migrant Farmworker Housing (MFH)

Change of a License:

Transfer of Ownership:

Please notify us before you transfer ownership or change the Uniform Business Identifier number.  A letter regarding the change of ownership information can be mailed, emailed, or faxed to the Department of Health and needs to include the following information:

  • The full name and address of the current licensee and prospective licensee.
  • The name and address of the currently licensed housing site and the name under which the migrant farmworker housing will operate.
  • The date of the proposed change.

The prospective new owner must let us know about the change.  We will need an application filled out and returned prior to operation.

Return the application packet with:

  • The licensing fee as specified in WAC 246-358-990 or WAC 246-361-990 for Cherry Harvest Camps, and
  • A copy of your Master Business License (also known as Uniform Business Identifier (UBI) number).  If you would like more information about Master Business License/Uniform Business Identifier (UBI) number, we suggest you contact the Washington State Department of Licensing (DOL) at their website (http://www.dol.wa.gov/contact.html) or call them at (360) 664-1400.

The application packet needs to be filled out completely.  Include a check or money order made payable to "Department of Health" and other documents as requested.  Please send to:

A license will be issued upon satisfactory completion of the following:

  • Initial application,
  • License fees,
  • Verification of approved water samples by Washington State Department of Health Drinking Water Program, and
  • On-site inspection conducted and the Department is satisfied the facility is in compliance with the law and rules, or if violations exist, corrections to them are satisfactorily addressed.
