Residential Treatment Facility

Changing a License:

*  Transfer of Ownership:

You will need to notify us at least sixty days prior to transferring the ownership of your property or the Uniform Business Identifier number.

.  A residential treatment facility license is not transferable.  Send us a letter and include the following information :

  • The full name and address of the current licensee and prospective licensee.
  • The name and address of the currently licensed residential treatment facility, and the name under which the transferred residential treatment facility will operate.
  • The date of the proposed change.
  • Plans for preserving resident records.
  • Other information as required by the department.

The prospective new owner must let us know about the change and will need to complete an initial application process.

*  Change of License Service Categories, Number of Resident Beds, Location or Use of Rooms, Business Name, or the Physical Structure of the RTF:

If you are making any of these changes to your license, you will need to notify us at least sixty days prior and submit:

  • A letter explaining the change.
  • If the change increases the total number of beds you will be offering, the facility must pay the additional fees under chapter 236-227-990 WAC.
