Applications The Certificate of Need (CoN) program is a regulatory process that requires certain health care providers to obtain state approval before building certain types of facilities or offering new or expanded services. Program staff is available to provide technical assistance to you before submitting your application. Staff is also available to help you determine if a Certificate of Need is necessary. The Department of Health and Certificate of Need are currently revising this Certificate of Need applications. Expectations are that the appropriate forms will be completed and available in the coming months. We apologize for any inconvenience. In the interim, please contact the program at 360-236-2955 for a copy of the appropriate forms.
Applicant Resources Certificate of Need applications require a degree of detail to be complete. In an effort to provide additional resources outside of our office staff, the Department of Health provides the following list as a convenience to you. This listing is not intended to be an exclusive list of all consultants available in the area of Certificate of Need services. By providing these names, the Department of Health is not indicating an endorsement. The Department of Health, nor the Program, will not involve itself in any issues relating to dissatisfaction regarding a consultant's performance
