Licensing by Examination:
An applicant must have passed the National Boards Parts I and II
and any state or regional dental testing examination. Exam results
are accepted for five years immediately preceding application for
state licensure. Applicants must also successfully complete the
state Jurisprudence examination, which is mailed to the applicant
once the application for licensure is complete. The Jurisprudence
examination is multiple choice and designed to familiarize the
applicant with the law relating to the practice of dentistry.
It is given in the open-book format.

Foreign Trained Dentists:
Must have satisfactory evidence of the successful completion
of at least two years of pre or postdoctoral academic years
of dental school education at a dental school accredited by the
American Dental Association and a certification by the dean of that school
that the candidate has achieved the same level of didactic and
clinical competence as expected of a graduate of that school.
Licensing without Examination/Licensed in Another State:
In 2003, RCW 18.32.215 was passed by the
legislature. This law allows an applicant holding a valid license
and currently engaged in the practice of dentistry in another
state to be granted a license without examination required by
this chapter, on the payment of any required fees, if the
applicant is a graduate of a dental college, school, or dental
department of an institution approved by the commission under
RCW 18.32.040(1). The applicant must submit an application and fee,
submit proof of active practice in another state or states,
and provide information as necessary pertaining to the conditions
and criteria of the Uniform Disciplinary Act, 18.130 RCW.
Upon completion of the application process, applicants will be
mailed a multiple choice Jurisprudence examination designed to
demonstrate knowledge of the law pertaining to the practice of
dentistry. The examination is given in the open-book format.

Refund of fees.
Fees submitted with applications
for initial credentialing, examinations, renewal, and other fees associated
with the licensing and regulation of the profession are
nonrefundable.

Making payments.
(1) Make checks or money orders payable to the department of health.
(2) Practitioners should include their credential number on the check, draft or money order.
(3) Applicants should include profession for which they are applying on the check, draft or money order.
