Expanded Function Dental Auxiliary (EFDA) School/State Approval

To qualify for an expanded function dental auxiliary (EFDA) license in this state all applicants must show completion of a commission approved EFDA education program before qualifying for a license. (RCW 18.260)

When your application is received, it will be initially reviewed for completeness. At that time, any additional information or materials deemed necessary will be requested. The application will then be provided to three Washington State Dental Quality Assurance Commission members for review. The reviewing members will report their findings to the full commission at the next available commission meeting. The commission will then vote to either deny, approve, or conditionally approve the application.

Before your program is approved, the commission will conduct a site-review. The site-review will consist of a visit to the school by representatives of the commission.

How to complete your application:

To assist the commission in making a timely decision, only completed applications submitted will be considered. The commission has the final authority to determine if the information is complete and if it meets the established standards.

To be considered complete, your application must be presented:

At least four weeks prior to commission meeting date to be considered at that meeting. With four copies of application OR one electronic copy (pdf OK).
