New forms required under law (18.06.130 RCW and 18.06.140 RCW)

The Department of Health developed sample forms for East Asian medicine practitioners to use to inform the public of their scope of practice and qualifications. The practitioner must fill out this form and give to each patient in writing prior to or at the time of the initial patient visit.

The department developed requirements for the written waiver for East Asian medicine practitioners to use when the practitioner sees a patient with a potentially serious disorder.
