The Office of Emergency Medical Services and Trauma System wants to improve the availability of products to our customers.  The products listed below may now be ordered by the method(s) indicated.

OEMSTS documents may be obtained from our Publications and Reports page.

Forms requiring payment must be completed on your computer screen and will calculate the payment you must include with your order, based on the items and the number of the items you request.  These forms must be printed out and sent via U.S. Mail to the address provided.

Forms that do not require payment must also be completed on your computer screen and printed.  They may be sent via U.S. Mail to the address provided, or faxed to the number provided.

NOTE:  EMS products such as Pins, Patches, and pocket sized Protocols and Trauma Bands are no longer available.    Protocols may be obtained on the publications page. 

EMS-NO CPR Forms are no longer available.  The new form is entitled "Physicians Orders for Life Sustaining Treatment (POLST)".  For more information, visit the POLST, Resuscitation (DNR) Orders page.
