EMS and Trauma Systems -
Develops and enforces rules, regulations and  standards for the education and certification of EMS personnel providing emergency  prehospital patient care care.

Application forms provided can no longer be filled out on the computer and printed.  Please print out the form, complete the application and obtain the required signatures.


Initial Certification
Initial certification is the process when a person has completed a Washington State DOH-approved training course and is applying for certification, at any level, for the first time.

Recertification      Online Renewal
A currently certified EMS provider within six months of their certification expiration date, requesting certification at the same level, for a second or subsequent time. Applicants can complete this paper application or complete their recertification through the Online Renewal process.

Reciprocity
Applying for Washington State EMS provider certification based on current EMS provider certification from another state or with the National Registry of Emergency Medical Technicians.

Challenge Educational Requirements for Certification
Applying for certification based on possession of a current health care provider credential and proof of education equivalent to the knowledge and skills for the level of certification.

Reissuance of Expired Certification
EMS providers with an expired certification may re apply for certification by meeting the reissuance requirements.

Reinstatement of a suspended, modified or revoked Washington State EMS Certification

Personal Status Change     Online Changes
Personal Status Change is the process when a person's name, address, or agency changes.
