Step 2:  Complete the Initial Certification Application
This form may be filled out on your screen, however, it MUST be printed out to obtain
the necessary signatures.

EMT-Basic

Step 1:  Obtain Emergency Medical Technician training

Step 2:  Complete the Initial Certification Application
This form may be filled out on your screen, however, it MUST be printed out to obtain
the necessary signatures.

Step 3:  Provide the information to this office

 

Intravenous Therapy Technician

Step 1:  Obtain Intravenous Therapy Technician training

Step 2:  Complete the Initial Certification Application
This form may be filled out on your screen, however, it MUST be printed out to obtain
the necessary signatures.

Step 3:  Provide the information to this office

 

Airway Management Technician

Step 1:  Obtain Airway Management Technician training

Step 2:  Complete the Initial Certification Application

IV/Airway Technician

Step 1:  Obtain IV/Airway training

Step 2:  Complete the Initial Certification Application

Poison Information Specialist

This credential is ONLY for individuals employed by the Washington State Poison Control Center and required to be certified as a Poison Control Specialist.
