Initial EMS Certification in Washington State

Initial certification is the process  when a person has completed a Washington State DOH-approved training course and is applying for certification, at any level, for the first time.  In addition, it is the process for credentialing Poison Control Specialists

This process applies to the following EMS certification Levels.   

  • Emergency Medical Responder (EMR)
  • Emergency Medical Technician (EMT)
  • Advanced EMT (AEMT)
  • Paramedic

Application forms provided can no longer be filled out on the computer and printed.  Please print out the form, complete the application and obtain the required signatures.

All EMS Provider Certification Levels

Step 1:  Complete a Washington State approved EMS training course and meet the requirements provided in the application packet.

Step 2:  Complete the Initial Certification Application
This form  MUST be printed out to complete and to obtain the necessary signatures.

Step 3:  Provide the information to the address provided above.

Poison Information Specialist

This credential is ONLY for individuals employed by the Washington State Poison Control Center and required to be certified as a Poison Control Specialist.

Step 1:  Complete the Initial Certification Application
This form  MUST be printed out to complete and to obtain the necessary signatures.

Step 2:  Provide the information to the address provided above.
