Reissuance or Reinstatement of an EMS Provider Certification
EMS providers with an expired certification may re apply for certification by meeting the reissuance requirements.

EMS Providers whose EMS certification has been modified, suspended or revoked may petition for reinstatement as provided in RCW 18.130.150.  

This process applies to the following EMS certification Levels.   

  • Emergency Medical Responder (EMR)
  • Emergency Medical Technician (EMT)
  • Advanced EMT (AEMT)
  • Paramedic

Reinstatement of a suspended or revoked EMS Certification

Application forms provided can no longer be filled out on the computer and printed.  Please print out the form, complete the application and obtain the required signatures.

Reissuance of an Expired EMS Provider Certification

Step 1:  Complete all the requirements provided in the application packet.

Step 2:  Complete the Recertification Application
This form  MUST be printed out to complete and to obtain the necessary signatures.

Step 3:  Provide the information to this the address provided above.
 

Reinstatement of a suspended, modified, or revoked EMS certification

  • Provide proof of completion of all requirements identified by the departmental disciplinary authority; and
  • Meet the reissuance requirements above.
