Some Facilities May Qualify for
Refunds
We have interpreted the application
requirements for ambulatory surgical facilities (ASF) to make the
process more clear. With these changes, some facilities may be eligible
to receive a full or partial refund of their fees.
An ambulatory surgical facility is any facility that performs outpatient
surgeries and uses general anesthesia. Such facilities are described in
state law (RCW 70.230.010(1) and RCW 70.230.040(3)). Facilities and
offices that do not use general anesthesia do not need this license and
may not obtain this license.
If you do not require this license, you may request a refund. Please
send us a written request stating you do not use general anesthesia and
wish to have the license fee refunded. This letter needs to be signed by
the requestor and mailed to DOH, PO Box 47877, Olympia, WA 98504. We
will refund unnecessary application fees.
If your facility is not licensed as an ambulatory surgical facility but
does use medications, you may need a DEA registration. Without an
ambulatory surgical facility license, you may need a Health Care
Entity-Pharmacy license.
If you paid a fee for an ASF license in lieu of renewing the health care
entity license, we will adjust the payment for the ASF license. This
will cover the $265 renewal fee for the health care entity license. The
remainder of the ASF license payment will be refunded. Please indicate
whether you need a health care entity license in your written refund
request. |
