The following is a description of the responsibilities of both parties,
the Project Sponsor, and the CRS team.
Project Sponsor Responsibilities
The project sponsor, usually the administrator of the health or
residential care facility/organization, is responsible for ensuring that:
- Construction is planned and designed in accordance with the
requirements established by DOH and/or the Department of Social and
Health Services (DSHS). While the planning and designing may be
delegated or contracted to an Architectural and /or Engineering firm,
the project remains the ultimate responsibility of the project
sponsor.
- The project documents comply with the Certificate of Need issued, if
a Certificate of Need was required for the project. (WAC
246-310)
- Requests for exemptions for nursing homes and boarding homes are
submitted by the project sponsor to the Director of Residential Care
Services (see Exemptions).
- Requests for substitutions and/or exemptions for projects, other than
Nursing Homes & Boarding Homes, are submitted to the Director of
Facilities and Services Licensing Division (see Exemptions).
- Responses are given to each review comment accompanied with two
copies of revised drawings. Answers to review
comments should also be complete, descriptive, and easily relatable to
the corrected drawings.
- Modifications, if desired, have been made by submitting two copies
of the revised drawings, addenda, change order, or supplemental
drawings/sketches.
- All submittals and correspondence shall be forwarded through the
project sponsor or identified designee, with the exception of shop
drawings. Shop drawings may be submitted by the installing contractor
(e.g., sprinkler, fire alarm, landscape, nurse call, special egress
control devices and kitchen hood and duct automatic fire extinguishing
systems).
Project Documentation
The following are integral parts and requirements for a project
submittal. The lack of any one requirement renders the submittal
incomplete and not reviewable. Incomplete submittals may be returned to
the sponsor without action.
- A properly completed Application
For Construction Review/Consultation form and payment of the
applicable fee.
- A written functional program in the initial submittal describing the
functions reflected in the design documents.
- A meal menu when remodel or new construction includes a kitchen.
- Drawings, Specifications and other documents as required to fully
define the project. All required documents shall be submitted
together, related, and coordinated. Each submittal shall be identified
with facility name and the CRS assigned project number. If applicable,
the phase of the project should be noted.
- Construction documents and specifications shall be signed and
stamped by a licensed architect or engineer (WAC
246-320-505, Hospital Design;
WAC
388-78A-070, Boarding Home Design; and WAC 388-97-400,
Nursing Home Design).
- Nursing and boarding home facility construction requires application
for an appropriate license at least 90 days prior to the anticipated
construction completion date. Proposed Nursing & Boarding homes
obtain licensing approval from the DSHS, Residential Care Services
Division.
- Health and residential care facility construction (all facilities
other than nursing and boarding homes) requires application for
license and/or notification of completion to the appropriate licensing
section at DOH at least six weeks prior to planned occupancy.
- Obtain approvals from the local building department and other
required local approvals.
CRS Team Responsibilities
- Take immediate action to record the project material received.
- Set up complete submittal for review on a "first-come,
first-serve basis."
- Return incomplete project submittals to the sponsor when information
received is incomplete, or is not clear and legible.
- Return incomplete and/or unsigned application
forms to sponsor without action.
- Return submitted documents when an application
and review fee have not been received.
- Verify the project against CON documentation, if appropriate.
- Render a complete and impartial review without unnecessary delay.
- Review for conformance to the rules and regulations of DOH and/or
DSHS. Each discrepancy referenced to the appropriate WAC.
- Review and recommend action on requests for exemptions, and for
approval of substitutions, as well as alternate material and methods.
- Advise appropriate department licensing staff of final approvals.
