Residential Treatment Facility

Application Requirements:

Any facility meeting the definition and planning to operate a residential treatment facility business, or currently operating such a business without a license, should contact our office immediately.

Contact our office by mail, telephone or e-mail to request that an application be sent or click on application link below. 

Application              Fee

The application needs to be filled out completely, include a check or money order made payable to "Department of Health.," and other documents as requested.  Please send to:
