Initial Application: Administration Processing and Application Fee $155An Annual Fee of $144.60 for each licensed bed Renewal Application: An Annual Fee of $144.60 for each licensed bed Late Fee: $25 per bed/per day past renewal date not to exceed $500 Change of Ownership Application: Administration Processing and Application Fee $155An Annual Fee of $144.60 for each licensed bed Follow-up Compliance Survey Fee: A complete on-site survey fee resulting from a substantiated complaint $1000
