Residential Treatment Facility

Initial Licensure:

A person must have a current license issued by the department before operating or advertising a residential treatment facility.  A license is effective for one year from the date of issuance.

To get an initial license you must send an application packet to us.  It has to get to us at least 60 days before you want to open. 

The application packet includes:

  • An application form that we will provide you.
  • The licensing fee specified in WAC 246-337-990.
  • Copy of Master Business License also known as Uniform Business Identifier (UBI) number .  If you would like more information about how to obtain a Master Business License/Uniform Business Identifier number, we suggest you contact the Washington State Department of Licensing (DOL) at their website (www.dol.wa.gov/business/) or call them at (360) 664-1400.
  • Room list identifying resident rooms, dimensions, and calculated square footage of each room, and number of approved bed spaces.
  • Reduced floor plan on letter size paper with identification of each room within the facility.
  • Criminal history background check and disclosure statement for those named in the application in accordance with RCW 43.43.
  • Other information as requested by the department.

New Construction:

Before you begin building or remodeling, you will need to notify Department of Health, Construction Review Section (CRS) about your plans.  Contact them at (360) 236-2944 or visit their website.
