Full Certification Requirements

  • Masters or doctoral degree, graduate semester hours and graduate quarter hours from institutions offering secretary approved programs.
  • Transcripts of all graduate work.
  • 2000 hours of treatment and evaluation experience. (These 2000 hours must include 250 hours of evaluation experience and 250 hours of treatment experience.)
  • 50 hours of formal conferences, symposia or seminars related to treatment and evaluation of sex offenders or abuse victims (obtained within the last three years.)
  • Underlying credential.
  • Successful completion of state examination.
  • Completion of a minimum of four clock hours of AIDS education.

Application Instructions for Full Certification
The application must be completed and include the following documentation and information:

  • Please review WAC 246-930-030 and 040 for Education Requirements.
  • Please type or print clearly on all application forms. Be aware that your application is a public record and may be released upon written request. Note: The address entered on the application form is your address of record. All correspondence will be sent to this address and it will appear on your certification. Official transcripts of all graduate work must be sent directly from the institution where the degree was earned to the Department of Health.  Proof of graduate semester hoursOR graduate quarter hours must be sent directly from the institution to the Sex Offender Treatment Program.
  • Applicants licensed or certified in another state or jurisdiction requesting endorsement must complete the upper portion of the Out of State Verification or Certification and forward the form to the jurisdiction of licensure or certification for completion. The licensing or certifying agency may then forward the form to the Department of Health, Sex Offender Treatment Program.  The following fees must accompany the application: Application ∓ exam fee $500.00 + Certification fee $100.00 =$600.00

Requirements for Affiliate Certification

  • Masters or doctoral degree, graduate semester hours and graduate quarter hours from institutions offering secretary approved programs.

  • Transcripts of all graduate work.
  • Supervision contract.
  • Underlying credential.
  • Successful completion of state examination.
  • Completion of a minimum of four clock hours of AIDS education.

Application Instructions for Affiliate Certification
The application must be completed and include the following documentation and information:

  • Please type or print clearly on all application forms. Be aware that your application is a public record and may be released upon written request. Note: The address entered on the application form is your address of record. All correspondence will be sent to this address and it will appear on your certification.
  • Official transcripts of all academic work must be sent directly from the institution where the degree was earned to the Department of Health.
  • Proof of graduate semester hours OR graduate quarter hours must be sent directly from the institution to the Sex Offender Treatment Program.
  • You and your supervisor must sign a written contract.
  • The following fee must accompany the application: Application, exam ∓ Certification fee of $200.00.

Examinations

Examinations will be given on an as needed basis.  All examinations will be given at a time and place determined by the Secretary of the Department of Health.   Click below for directions to the Department of Health.

Certification Renewal

Certifications for both fully certified and affiliate providers expire annually on the providers date of birth. Renewals are considered timely if postmarked on or before the expiration date. Certifications that are allowed to expire are subject to an additional late renewal penalty fee. Courtesy renewal notices are sent approximately 30 days prior to the expiration date; however, the provider is responsible for renewing his or her certification prior to the expiration date.
