Apply for Registry Access Authorization

Do you know your patients' final wishes?

Health care practitioners and facilities such as hospitals, nursing homes, hospice agencies, and other licensed providers may apply for access to the registry by completing a simple application form (347 KB PDF). By gaining authorization to the registry, your facility will have an organized and easy way to store and retrieve your patients’ end-of-life treatment wishes. The registry is free of charge and may be accessed 24 hours a day. Learn more about registry benefits.

Once the application is completed, mail or fax it to the Washington State Department of Health for approval. The application contains the mailing address and fax number. Your application will be reviewed within 3 to 5 business days.  Once approved, you will receive a letter containing your unique access code and instructions for retrieving documents.
