will find a sticker on their driver’s license or identification card. You can
also access the registry and search by name and birth date if no
identification is present.

preferences which will help you in fulfilling their wishes.

End-of-life discussions with family members are difficult. The registry will
enable you to talk about the patient’s wishes with confidence and concrete
information.

Emergency contact information for family or next-of-kin will be stored in the
registry along with the living will documents in order for family to be
contacted immediately in an emergency.

their documents.
