Authorized health care providers may access the Washington State Living Will Registry 24 hours
a day. Health care facilities interested in gaining access to the registry must first complete
a simple
application form
(303 KB PDF) and submit it to the Washington State Department of Health for
approval. Once approved, you will receive a letter containing your unique organization ID
number, access code, and instructions on how to use the registry.
