  Authorized health care providers may access the Washington State Living Will Registry 24 hours a day. Health care facilities interested in gaining access to the registry must first complete a simple application form (303 KB PDF) and submit it to the Washington State Department of Health for approval. Once approved, you will receive a letter containing your unique organization ID number, access code, and instructions on how to use the registry.

The Living Will Registry is managed by the Washington State Department of Health. The department exclusively selected the U.S. Living Will Registry company to maintain our Washington State Living Will Registry.
