employees, contractors, students, and volunteers who access or have access
to medical information will sign a Confidentiality Statement annually that
such information will be held in confidence with the understanding that any
violation of the confidentiality of client information may result in
disciplinary action up to and including dismissal, dissolution of
contractual agreement and possible monetary fines or imprisonment. Purpose: 1.
To establish a consistent policy for all employees, contractors,
students, and volunteers accessing or having access to client health
information to sign a Confidentiality Statement. 2.
To ensure confidentiality of all client health information contained
within the Health Department regardless of method of storage. Procedure: 1.
All new and currently employed staff, contractors, students and
volunteers within the Health Department dealing with client health
information are required to sign a Confidentiality Statement on an annual
basis. 2.
The process of signing the Confidentiality Statement will become a
part of the Health Department orientation for those staff having access to
health care information and will occur on the first day of employment or
when contractors, students and volunteers begin their affiliation with the
Health Department and will be witnessed by the individual conducting this
part of the orientation. 3.
Currently employed staff will sign the first working day of the new
year and will be witnessed by their manager or by their manager's designee. 4.
The Confidentiality Statement will remain in the individual's
personnel file or with the contract as appropriate. 5.
Confidentiality training on the handling of sensitive information
will be a part of the Health Department's orientation and will continue on
an annual basis.
