Purpose of Reporting and Surveillance
  • To ensure the safest strategies of vaccine use and to further reduce the rare risks associated with vaccines.

Reporting Requirements
  • Health care providers: notifiable within 3 workdays
  • Hospitals: notifiable within 3 workdays
  • Laboratories: no requirements for reporting
  • Local health jurisdictions: notifiable to DOH Immunization Program within 7 days of case investigation completion or summary information required within 21 days
