Welcome to the Washington State Department of Health site for public record disclosure. This site provides information on:
How to make a request?
To make a Public Record Disclosure request, you may e-mail, call, write, fax or ask for the records in person.
The listing below shows e-mail, postal and phone information for each Public Record Disclosure Coordinator.
If you believe records may be in one or more division or office, please send it to just one. Public Record
Disclosure Coordinators check with each other when records are likely to be in more than one place. If you know
that records may be in different places, please include that information in your request.
To e-mail your request, click on the name of a Public Record Disclosure Coordinator for the division or office
where you believe the records are. An e-mail to that Coordinator will appear on your screen.
If you want to make the request in person, contact the Public Record Disclosure Coordinator for directions to
the office.
For us to respond as quickly as possible, please provide as much information about the records as you can. We
also need information about how to contact you if we have questions and where to send the records.
How to receive records
You may receive records responsive to your request in three ways. You may choose to receive them:

As paper copies we send you in the mail.
- Currently, if there are 50 or fewer pages, there is no charge.
- If there are 51 pages or more, we charge 15 cents per page for all the pages.
- We do not charge a separate fee to cover postage costs.

Electronically (as e-mail attachments, on a disk or other media).
- Currently we do not charge a fee when we provide records as an e-mail attachment. Programs may charge for media other than e-mail
attachments.
- Please note that some responses contain many pages. We send approximately 100 pages of records as an e-mail attachment. We will
send multiple e-mails with that size attachment to provide you all of the records. Your machine must be able to handle that amount of information in an attachment.

By inspecting the records at the DOH Office where the majority of the records are located.
- You must arrange for inspection ahead of time with the public disclosure contact who responds to your request.
- Inspection times are from 9AM-noon and 1-4 PM weekdays, except days that are state recognized holidays. You may be able to arrange an
inspection during a noon hour. It depends on whether the staff in the office are available at noon.
- After inspection, you may ask us to make copies. We will either send them to you electronically, or make paper copies.
