State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA ¾ The Health Professions Quality Assurance Office (HPQA) of the Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in the state.

HPQA is responsible for protecting and enhancing the health of the people of
Washington State by assuring access to safe, competent health care providers. HPQA personnel work in partnership with 12 boards, four commissions, and nine advisory committees to set licensing standards for 55 health care professions (e.g., medical doctors, nurses, counselors).

Information on Washington’s health care providers is now available online at our new Provider Credential Search Web site (https://fortress.wa.gov/doh/providercredentialsearch/). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling HPQA, 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

There is currently an opening on the Orthotics and Prosthetics Advisory Committee for a licensed physician specializing in orthopedic medicine or surgery or physiatry. Please contact 360-236-4947 for information.

Cowlitz County

In June 2004 the Nursing Assistant Program revoked the certification of nursing assistant Kisha Young (NC10065164) for five years. Young diverted controlled substances from her workplace.

King County

In June 2004 the Unlicensed Practice Program issued a Notice of Intent to Issue a Cease and Desist Order against Patricia Styles. Allegations against her include performing massage to the public for compensation and advertising herself as a clinical nurse. She does not hold a license to practice massage or nursing.

In June 2004 the Nursing Assistant Program revoked the registration of Tammy M. Williamson, a nursing assistant (NA00124253), for 10 years. Williamson pled guilty to theft.

Lewis County

In June 2004 the Nursing Assistant Program revoked the certification of nursing assistant Kelly Jepsen (NC10071658) for 10 years. Jepsen stole checks from a patient and used them for personal purchases.

Pacific County

In June 2004 the Nursing Assistant Program revoked the certification of Kim M. Crockford, a nursing assistant (CA00037047), for five years. Her application to practice as a nursing assistant is denied, and she may not reapply for five years. Crockford concealed theft convictions on her application.

Pierce County

In June 2004 the Nursing Assistant Program revoked the certification of nursing assistant Sharon Batiste (NC10062166) for 10 years. Batiste abused a patient who was a vulnerable adult.

In June 2004 the Nursing Commission indefinitely suspended the license of registered nurse Dannielle L. Thorpe (RN00141993). Thorpe failed to comply with the terms and conditions of a prior order.

Spokane County

In June 2004 the Pharmacy Board suspended the license of pharmacist Patrick Ashley (PH00014275) for 60 days. After this time, his license will be placed on probation for five years. He is prohibited from managing a pharmacy or supervising interns, must practice under supervision and must submit reports to the board. Ashley exerted unauthorized control over approximately $1,700 belonging to his employer.

In June 2004 the Nursing Commission suspended the license of registered nurse Glenna J. Jacobson (RN00110818) for one year. On several occasions, she failed to document administration or wastage of medication intended for patients.

In June 2004 the Nursing Commission indefinitely suspended the license of registered nurse Lynn Dee Monroe (RN00102774). Monroe forged prescriptions for a controlled substance from a prescription pad she stole from her workplace.

Stevens County

In June 2004 the Nursing Assistant Program revoked the certification of nursing assistant Christina M. Williamson (NC10062935) for five years. She pled guilty to theft.

Out of State

(Many health care providers hold licenses in more than one state. Department of Health receives information each month from national groups on disciplined health care providers. When another state’s regulatory group takes actions, the equivalent board/commission/program in Washington files unprofessional conduct charges against the provider’s Washington credential.)

California: In June 2004 the Nursing Commission suspended the license of Shirley R. Freeman, a registered nurse licensed in Washington (RN00120628). Freeman may not seek modification of the order and/or license reinstatement until she complies with the terms and conditions of a prior order which she has failed to comply with.

Texas: In June 2004 the Board of Osteopathic Medicine and Surgery indefinitely suspended the license of Robert G. Garmon, an osteopathic physician and surgeon (OP00000710). In 2002 the Texas Board of Medical Examiners entered an Agreed Order with Garmon. He has restrictions against his Texas license for five years.
