State revokes, suspends licenses, certifications, registrations of health care providers
OLYMPIA ¾ The Health Professions Quality Assurance Office (HPQA) of the Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in the state.
HPQA is responsible for protecting and enhancing the health of the people of
Washington State by assuring access to safe, competent health care providers.
HPQA personnel work in partnership with 12 boards, four commissions, and nine
advisory committees to set licensing standards for 55 health care professions
(e.g., medical doctors, nurses, counselors).
Information on Washington’s health care providers is now available online at our new Provider Credential Search Web site (https://fortress.wa.gov/doh/providercredentialsearch/). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling HPQA, 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
There is currently an opening on the Orthotics and Prosthetics Advisory Committee for a licensed physician specializing in orthopedic medicine or surgery or physiatry. Please contact 360-236-4947 for information.
In September 2004 the Nursing Commission indefinitely suspended the license of registered nurse Francisca Arthur (RN00134565). Arthur failed to comply with the terms and conditions of a prior order and diverted controlled substances from her workplace for her non-therapeutic use.
In September 2004 the Nursing Commission indefinitely suspended the license of registered nurse Sara J. Smith (RN00139946). Smith diverted controlled substances from patients.
In September 2004 the Nursing Assistant Program revoked the registration of Devon N. Bauer, a nursing assistant (NA00161049), until she can demonstrate she is competent and able to practice with reasonable skill and safety. Bauer failed to comply with the terms and conditions of a prior order.
In September 2004 the Dental Commission placed the license of dentist Young S. Lee (DE00008790) on suspension for three years. All but one month of the suspension will be stayed. Lee must complete continuing dental education, have his staff complete a risk management course, identify a work-site monitor and pay a $10,000 fine. He aided and abetted the unlicensed practice of dental hygiene and had deficiencies in an infection control inspection at her place of practice.
In September 2004 the Mental Health Counselor Program indefinitely suspended the registration of Gregory E. Pritchett, a mental health counselor (LH00004046). Pritchett sent a patient three emails with child pornography attached.
In September 2004 the Health Care Assistant Program revoked the credentials of health care assistant Rita L. Turner (HC00005046, HC00128431, HC00130733) for three years. Turner has used various social security numbers on health care assistant application forms and other publicly recognized documents. She also was convicted of theft. Rita L. Turner is also known as Rita J. Turner, Rita Turner and L.R. Turner. The order against her was corrected on September 30, 2004.
In September 2004 the Chiropractic Commission revoked the license of chiropractor Douglas D. Duncan (CH00001300) for five years. Duncan had sexual contact with patients.
In September 2004 the Nursing Commission revoked the license of registered nurse Jeffery B. Alexander (RN00129397) for five years. He failed to comply with the terms and conditions of a prior order.
In September 2004 the Nursing Commission indefinitely suspended the license of Timothy Anderson, a licensed practical nurse (LP00042476). In May 2002 the Idaho Board of Nursing denied his application for reinstatement of his license.
In September 2004 the Nursing Assistant Program revoked the certification of Wendy Billings-Keblbek, a nursing assistant (NC10072293), until she can demonstrate she is competent and able to practice with reasonable skill and safety. She failed to comply with the terms and conditions of a prior order.
In September 2004 the Health Care Assistant Program and the Nursing Assistant Program indefinitely suspended the certifications of Richard H. Hanson, Jr., a health care assistant and nursing assistant (HC00140926, NC10065586). He stole travelers’ checks from a patient, cashed the checks and spent the money.
In September 2004 the Massage Therapy Program amended a Final Order against massage therapist Anthony Masiello (MA00004022). His license is revoked for five years for sexual contact with patients.
In September 2004 the Unlicensed Practice Program issued a Cease and Desist order against Nancy McGillen. She must immediately cease and desist from representing herself and performing duties of a registered nurse. McGillen does not have a license to practice nursing.
In September 2004 the Nursing Commission indefinitely suspended the license of registered nurse Margaret Brodie (RN00124850). Brodie abused drugs and diverted controlled substances from her workplace.
In September 2004 the Nursing Assistant Program indefinitely suspended the certification of nursing assistant Shannon Paterson (NC10049219). Paterson submitted a urine sample that tested positive.
(Many health care providers hold licenses in more than one state. Department of Health receives information each month from national groups on disciplined health care providers. When another state’s regulatory group takes actions, the equivalent board/commission/program in Washington files unprofessional conduct charges against the provider’s Washington credential.)
Canada: In September 2004 the Nursing Commission indefinitely suspended the license of Shannon M. Stephens, a registered nurse licensed in Washington (RN00123727). She diverted a controlled substance from her workplace.
Oregon: In September 2004 the Unlicensed Practice Program issued a Cease and Desist Order against Phillip E. Leveque. He must immediately cease and desist from conduct constituting practice of medicine in Washington. He must also pay a $1,000 fine. Leveque, who is licensed to practice medicine in Oregon, completed and signed a form giving a patient medical authorization to possess marijuana for medical purposes in Washington.
