State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA The Health Professions Quality Assurance Office (HPQA) of the Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in the state. The information on the cases reported in this news release represents the status of each item on the date the news release was issued.

HPQA is responsible for protecting and enhancing the health of the people of Washington by assuring access to safe, competent health care providers. HPQA personnel work in partnership with 12 boards, four commissions, and nine advisory committees to set licensing standards for 55 health care professions (e.g., medical doctors, nurses, counselors).
 

Information on Washington» s health care providers is now available online at our new Provider Credential Search Web site (https://fortress.wa.gov/doh/providercredentialsearch/). The site includes information about a health care provider» s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. Benton County

In October 2005 the Nursing Commission indefinitely suspended the license of Kristen Gibson, a licensed practical nurse (LP00016394). She diverted controlled substances from her workplace for her own use.

Clark County

In November 2005 the Massage Program entered an Agreed Order with Nicholas A. Banks, licensed to practice massage (MA00004397). He agrees to surrender his license on allegations of inappropriate touching and disclosure of confidential information.

In November 2005 the Nursing Commission denied the reactivation application of Karyl R. Craig to practice as a licensed practical nurse (LP00046309). Her license is placed on suspension for two years. Craig was convicted of assault and bail jumping.

King County

In October 2005 the Health Care Assistant Program revoked the certification of health care assistant Erika C. Adams (HC00142228) for five years. Her nursing assistant certificate was revoked in 2004 based on failure to comply with a prior order.

In October 2005 the Nursing Assistant Program revoked the registrations of nursing assistant Rebecka L. Fowler (NA00146780) for 10 years. While working at an assisted living facility, she stole money from a patient» s purse.

In October 2005 the Nursing Commission indefinitely suspended the license of registered nurse Jenon Laurene (RN00069211). She showed up to work under the influence of alcohol.

In October 2005 the Chemical Dependency Professional and Registered Counselor Programs indefinitely suspended the certifications of Paul A. Ortega, a chemical dependency professional and registered counselor (CP000001167, RC00017328). An audit of his practices found insufficiencies, including files that contained blank treatment plans, files lacking treatment plans and files lacking complete progress notes.

Kittitas County

In October 2005 the Nursing Commission suspended the license of registered nurse Valerie L. Marjerrison (RN000146675). She diverted controlled substances from her workplace for her own non-therapeutic use and failed to comply with a monitored treatment program.

Pierce County

In October 2005 the Hypnotherapist Program revoked the registration of hypnotherapist Ross E. Anderson III (HP10001032) for 20 years. He crossed professional boundaries with a client, including having sex with the client.

In October 2005 the Unlicensed Practice Program issued a Cease and Desist Order against Sung M. Im. He must immediately stop practicing as an unlicensed medical doctor and pay a $25,000 fine. Im is not licensed in Washington but operated a residential program in Eatonville where he examined and represented he would treat patients.

Skagit County

In October 2005 the Unlicensed Practice Program issued a Cease and Desist Order against Tina M. Floe. She continued to practice massage, including treating and billing patients, after her license was expired.

In October 2005 the Nursing Assistant Program indefinitely suspended the certification of nursing assistant Serina R.J. Newman (NC10017840). She practiced beyond the scope of practice and failed to keep a current address on file with the department.

Snohomish County

In October 2005 the Registered Counselor Program suspended the registration of registered counselor Jennifer L. Giese (RC00005439). She had a personal and sexual relationship with a client.

In October 2005 the Unlicensed Practice Program issued a Cease and Desist Order against Jaklynn Robertson. She must immediately stop practicing as an unlicensed emergency medical technician. Robertson sutured a person» s injuries.

Spokane County

In October 2005 the Nursing Assistant Program revoked the certification of nursing assistant Raymond C. Hughes (NC10038348) for 10 years. He pled guilty to raping a vulnerable child.

Thurston County

In October 2005 the Nursing Commission suspended the license of registered nurse Stacey Loewen-Hays (RN00148268) for two years. She diverted controlled substances from several workplaces for her own non-therapeutic use.

Walla Walla County

In October 2005 the Registered Counselor Program revoked the registration of registered counselor Rochelle E. Helms (RC00035749). She failed a drug test and associated and communicated with a client in an unprofessional manner.

Out of State

(Many health care providers hold licenses in more than one state. Department of Health receives information each month from national groups on disciplined health care providers. When another state» s regulatory group takes actions, the equivalent board/commission/program in Washington files unprofessional conduct charges against the provider» s Washington credential.)

California: In October 2005 the Unlicensed Practice Program issued a Cease and Desist Order against Bin "Sam" Xia requiring him to cease practice of massage. He does not have a license to practice massage in Washington. He performed massage on a client at a spa in Redmond. Bin "Sam" Xia is also known as Xia Bin.

Oregon: In October 2005 the Chemical Dependency Professional Program indefinitely suspended the certification of Anne M. Westrup, a chemical dependency professional licensed in Washington (CP00002392). She has a history of drug and alcohol misuse.
