OLYMPIA ¾ The Health Professions Quality Assurance Office (HPQA) of the Washington State Department of Health has taken disciplinary actions or has withdrawn charges against health care providers in the state. The information on the cases reported in this news release represents the status of each item on the date the news release was issued.
HPQA is responsible for protecting and enhancing the health of the people of Washington by assuring access to safe, competent health care providers. HPQA personnel work in partnership with 12 boards, four commissions, and nine advisory committees to set licensing standards for 55 health care professions (e.g., medical doctors, nurses, counselors).
Information on Washington’s health care providers is now available online at our new Provider Credential Search Web site. The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling HPQA, 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Asotin County
In October 2005 the Nursing Assistant Program charged nursing assistant Sarah
J. Draper (NA00168260) with unprofessional conduct. She pled guilty to
attempting to obtain a controlled substance through fraud and possession of a
controlled substance. Her pleas of guilty were based upon her theft of
prescription medication from her patients.
King County
In October 2005 the Dental Commission amended a statement of charges against
dentist Mark E. Anderson (DE00005689). He is being charged with allegedly
being unable to practice dentistry with reasonable skill and safety due to a
physical or mental condition.
In October 2005 the Unlicensed Practice Program issued a Notice of Intent to
issue a Cease and Desist Order against Great Wall Medical and Dental Group,
Inc. in Kent. The ownership and structure of the organization allegedly
violates the law and results in unlicensed practice of dentistry and unlicensed
practice of medicine.
In August 2005 the Pharmacy Board charged Ronald D. Hancock, a pharmacy
technician (VA00019174), with unprofessional conduct. He allegedly stole
merchandise from his workplace.
In October 2005 the Nursing Assistant Program reinstated the certification of
Leslie A. Somerville to practice as a nursing assistant (NC10025933).
In October 2005 the Pharmacy Board charged Arif Raymond Timmons, a
pharmacy technician (VA00053128), with unprofessional conduct. Timmons allegedly
diverted controlled substances from his workplace for his own use and the use by
others.
Mason County
In October 2005 the Nursing Assistant Program charged nursing assistant Larry
Witt (NC10081942) with unprofessional conduct. He allegedly engaged in
sexual contact with a patient.
Pierce County
In November 2005 the Nursing Assistant Program charged nursing assistant
Dolores L. Chandler (NC10062939) with unprofessional conduct. She allegedly
removed money from a patient’s wallet without permission.
In November 2005 the Radiologic Technologist Program placed the certification of
Jerrie D. Guillermo, a radiologic technologist (RT00005303), on a stayed
suspension for one year. He must comply with terms and conditions, including
paying a $5,000 fine. Guillermo made inappropriate sexual remarks to a patient,
gave unwanted assistance in helping the patient dress after an exam and hugged
the patient.
In November 2005 the Nursing Assistant Program charged nursing assistant
Deetta D. Smith (NC10068985) with unprofessional conduct. She allegedly
physically abused patients.
In October 2005 the Nursing Commission reinstated the license of Dannielle L.
Thorpe to practice as a registered nurse (RN00141993).
In November 2005 the Nursing Commission charged registered nurse Linda Wayman-Davis
(RN00071973) with unprofessional conduct. She allegedly failed to comply with
the terms and conditions of a prior order.
Snohomish County
In October 2005 the Nursing Assistant Program charged nursing assistant
Caroline R. Devi (NA00127984) with unprofessional conduct. She allegedly
failed to report a change in a patient’s condition.
In November 2005 the Nursing Assistant Program charged nursing assistant
Lisa R. Devore (NC10078352) with unprofessional conduct. She allegedly
physically abused a patient.
In October 2005 the Nursing Assistant and Health Care Assistant Programs charged
Setareh Eghterafi, a nursing assistant and health care assistant
(NC10014081, HC00135855), with unprofessional conduct. Allegations against her
include failing to follow physician’s order while treating patients.
In August 2005 the Nursing Commission charged Matthew Skews, a licensed
practical nurse and registered nurse (LP00041761, RN00114682), with
unprofessional conduct. He allegedly failed to comply with the terms and
conditions of a prior order.
Spokane County
In October 2005 the Nursing Commission charged Carrie L. Brewer, a
licensed practical nurse (LP00050994), with unprofessional conduct. She
allegedly falsified a patient’s medical records.
In November 2005 the Nursing Assistant Program charged nursing assistant
Shelly H. Harris (NC10076376) with unprofessional conduct. She allegedly
acted in a way that put emotional distress on a patient.
In October 2005 the Registered Counselor and Chemical Dependency Professional
Programs charged Julie A. Hipp, a registered counselor and chemical
dependency professional (RC00044552, CP00004582), with unprofessional conduct.
Allegations against her include falsely reporting and charting she met with a
client and misrepresentation or fraud.
In November 2005 the Nursing Assistant Program charged nursing assistant
reactivation applicant Rabecka C. Wilson (NC10058126) with unprofessional
conduct. She was convicted of second degree theft.
Thurston County
In October 2005 the Unlicensed Practice Program issued a Notice of Intent to
issue a Cease and Desist Order against Rachel Greenwood. She allegedly
drew patients’ blood and gave patients injections. Greenwood is not credentialed
to practice as a health care provider.
In October 2005 the Nursing Commission placed the license of registered nurse
Karon Hume Stiles (RN000768781) on probation for one year. She must comply
with terms and conditions. Stiles failed to meet the standard of practice while
caring for patients.
Whatcom County
In October 2005 the Registered Counselor Program charged registered counselor
Robyn L. O’Hara (RC00020104) with unprofessional conduct. She allegedly
failed to comply with prior limitations placed against her registration.
Yakima County
In November 2005 the Nursing Assistant Program charged nursing assistant
Nicole M. Gonzales (NC10064494) with unprofessional conduct. She was
convicted of negligent driving, possessing marijuana and minor in possession.
In October 2005 the Unlicensed Practice Program issued a Notice of Intent to
issue a Cease and Desist Order against Michelle Granger. She allegedly
represented herself a massage therapist and gave massages for a fee. She does
not have a license to practice massage.
In October 2005 the Board of Osteopathic Medicine and Surgery withdrew a
statement of charges against George S. Toliver (OP00001397).
Out of State
(Many health care providers hold licenses in more than one state. Department of
Health receives information each month from national groups on disciplined
health care providers. When another state’s regulatory group takes actions, the
equivalent board/commission/program in Washington files unprofessional conduct
charges against the provider’s Washington credential.)
California: In October 2005 the Medical Commission withdrew a statement
of charges against Kenneth G. Petty, a physician licensed in Washington
(MD00033716).
Indiana: In October 2005 the Physical Therapy Board charged Sharon L.
Worden, a physical therapist licensed in Washington (PT00002944), with
unprofessional conduct. Allegations against her include failing to document her
continuing education and failing to cooperate with a request from the board.
Oregon: In November 2005 the Nursing Assistant Program withdrew a
statement of charges against Katina M. Marrapodi, a nursing assistant
licensed in Washington (NC10045954).
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
