State disciplines health care providers

OLYMPIA ¾ The Health Professions Quality Assurance Office (HPQA) of the Washington State Department of Health has taken disciplinary actions or has withdrawn charges against health care providers in the state. The information on the cases reported in this news release represents the status of each item on the date the news release was issued.

HPQA is responsible for protecting and enhancing the health of the people of Washington by assuring access to safe, competent health care providers. HPQA personnel work in partnership with 12 boards, four commissions, and nine advisory committees to set licensing standards for 55 health care professions (e.g., medical doctors, nurses, counselors).

Information on Washington’s health care providers is now available online at our new Provider Credential Search Web site (https://fortress.wa.gov/doh/providercredentialsearch/). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling HPQA, 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

In December 2005 the Nursing Assistant Program granted the application of Victor M. L. Armijo to practice as a nursing assistant (CA00041966). His registration is placed on probation for one year and he must comply with terms and conditions. Armijo was convicted of conspiring to distribute cocaine.

Chelan County

In November 2005 the Registered Counselor Program charged Lisa R. Barker, an applicant to practice as a registered counselor (CA00007660), with unprofessional conduct. She was convicted of theft.

Clark County

In December 2005 the Veterinary Board of Governors placed the license of veterinarian Erich Bargar (VT00006850) on probation for one year. He must comply with terms and conditions. Bargar allowed an unlicensed person to perform veterinarian services, failed to ensure adequate documentation, failed to keep controlled substances in a locked cabinet and failed to maintain a log of controlled substances. 

Cowlitz County

In November 2005 the Registered Counselor Program charged Alfredo M. Diaz, an applicant to practice as a registered counselor (CA00007515), with unprofessional conduct. He was convicted of possessing cocaine. Alfredo M. Diaz is also known as Augustine Odon Tinoco.

King County

In December 2005 the Health Care Assistant Program vacated an Order revoking the health care assistant credential of Erika C. Adams (HC00142228).

In December 2005 the Health Care Assistant Program denied the application of Richard M. Beecham, to practice as a health care assistant (CA00010687). He may not reapply for five years. Beecham was convicted of burglary, driving with a suspended license and hit and run.

In November 2005 the Registered Counselor Program charged Alyson E. Brown, an applicant to practice as a registered counselor (CA00007651), with unprofessional conduct. Brown was convicted of driving under the influence, theft and minor in possession.

In December 2005 the Registered Counselor Program granted the application of Eric R. Daniels to practice as a registered counselor (CA00007190). His registration is placed on probation for two years and he must comply with terms and conditions. Daniels was convicted of robbery.

In November 2005 the Health Care Assistant Program placed the certification of health care assistant Tommy L. Grissette (HC00011421) on a stayed suspension for three years. He must comply with terms and conditions. Grissette was convicted of three counts of violating the uniform controlled substance act for forging prescriptions.

In December 2005 the Examining Board of Psychology amended a statement of charges against psychologist Philip A. Hirsch (PY00001117). He is being charged with unprofessional conduct for providing treatment to patients without a current license.

In November 2005 the Chiropractic Commission charged chiropractor Mark D. Rutherford (CH00002419) with unprofessional conduct. He allegedly failed to maintain appropriate patient records and failed to comply with prior limitation placed against his license.

In December 2005 the Surgical Technologist Program modified a prior order and granted the application of Melissa G. Venturine to practice as a surgical technologist (CA00001663). Her registration is placed on probation for one year and she must comply with terms and conditions.

In December 2005 the Massage Program denied the application of Carla A. Warren to practice as a massage therapist (CA00007268). She may not reapply for 10 years. Warren was convicted of theft and possession with intent to distribute marijuana.

Mason County

In November 2005 the Dental Commission amended a statement of charges against dentist Shirley O. Daniels (DE00007922). She is being charged with unprofessional conduct for allegedly providing treatment that failed to meet the accepted standard.

In November 2005 the Registered Counselor Program charged Bryan W. Parkhurst, an applicant to practice as a registered counselor (CA00007623), with unprofessional conduct. He was convicted of theft and unlawful possession of a controlled substance.

Pierce County

In November 2005 the Medical Commission charged physician John A. Kennedy (MD00008728) with unprofessional conduct. Allegations against him include inappropriately billing Medicare.

In November 2005 the Chemical Dependency Professional Program charged Paula J. Welsh, a chemical dependency professional (CP00002275), with unprofessional conduct. She allegedly crossed professional boundaries with a client.

Snohomish County

In December 2005 the Pharmacy Board charged pharmacist Florence A. Green (PH00018953) with unprofessional conduct. Allegations against her include misfiling prescriptions.

In November 2005 the Chemical Dependency Professional and Registered Counselor Programs charged John S. Hite, a chemical dependency professional and registered counselor (CP00004307, RC00042448), with unprofessional conduct. He allegedly failed to comply with the terms and conditions of a prior order.

In December 2005 the Nursing Commission charged Carolyn F. Oliver, a licensed practical nurse (LP00044223), with unprofessional conduct. She allegedly failed to comply with the terms and conditions of a prior order.

In November 2005 the Radiologic Technologist Program charged A. Brian Stambuk, an applicant to practice as a radiologic technologist (CA00003799), with unprofessional conduct. Allegations against him include practicing without a license, making inappropriate sexual remarks and exhibiting inappropriate sexual behavior towards a patient. Stambuk was also convicted of driving under the influence and assault.

Spokane County

In December 2005 the Dental Commission charged dentist Henry G. Kolsrud (DE00002069) with unprofessional conduct. Allegations against him include having an unclean facility, not sufficiently sterilizing instruments and failing to provide adequate training to staff.

Thurston County

In December 2005 the Nursing Assistant Program charged nursing assistant James A. De La Vergne (NC10060820) with unprofessional conduct. He allegedly took personal checks from a patient and forged and cashed them for more than $10,000. Vergne was convicted of theft.

Out of State  

(Many health care providers hold licenses in more than one state. Department of Health receives information each month from national groups on disciplined health care providers. When another state’s regulatory group takes actions, the equivalent board/commission/program in Washington files unprofessional conduct charges against the provider’s Washington credential.)

 

Oregon: In November 2005 the Examining Board of Psychology charged Edward A. Blaske, an applicant to practice as a psychologist in Washington (CA00003143), with unprofessional conduct. He was convicted of driving under the influence in Ohio. The Ohio board imposed restrictions on his license in that state.

 

Oregon: In December 2005 the Veterinary Board of Governors entered into an Agreed Order with Joseph M. Kiel a veterinarian licensed in Washington (VT00000476). His license is restricted and he must comply with terms and conditions. Kiel performed surgeries that resulted in post-operative complications.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
