State revokes, suspends licenses, certifications, registrations of health care providers
Department taking more steps to improve patient safety

OLYMPIA The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department» s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency» s Web site. Click on "Provider Credential Search" on the left hand side of the home page (www.doh.wa.gov). The site includes information about a health care provider» s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In August 2006 the Registered Counselor Program suspended the registration of Sandra G. Sandven, a registered counselor (RC00049757), for three years. Sandven used marijuana and methamphetamine.

In August 2006 the Health Care Assistant Program revoked the certification of health care assistant Michelle L. Williams (HC00141295) for five years. Williams wrote a doctor» s name on a prescription without the doctor» s authorization and was convicted of unlawful possession of a drug.

Chelan County

In August 2006 the Nursing Assistant Program revoked the certification of nursing assistant Christina L. Page (NC10011765) for three years. Page failed to record the administration of medication in a patient» s chart and stole several doses of the medication from the patient.

In August 2006 the Nursing Assistant Program indefinitely suspended the certification of Diane L. Pate, a nursing assistant (NC10031000). She consumed alcohol while on the job, left a patient unattended for several hours and misrepresented herself to her employer.

In July 2006 the Chemical Dependency Professional Program revoked the certification of Ronald E. Tribou, a chemical dependency professional (CP00004686) for five years. Tribou engaged in a personal and sexual relationship with a client.

Grays Harbor County

In July 2006 the Veterinary Board revoked the license of veterinarian Roy H. Haralson (VT00003069) for 10 years. He was convicted of child rape and possession of depictions of minors engaged in sexually explicit conduct.

King County

In August 2006 the Nursing Assistant Program revoked the license of nursing assistant Gloria J. Dixon (NC10005035) for making unauthorized purchases against a patient» s bank account. The purchases totaled more than $2,300.

In August 2006 the Nursing Commission indefinitely suspended the license of registered nurse Tanya C. Edge (RN00127512). She failed to comply with prior limitations on her license.

In August 2006 the Nursing Commission indefinitely suspended the license of Lynn Farrell, a licensed practical nurse (LP00020690). She failed to comply with a prior order against her license.

In August 2006 the Dental Commission revoked the license of dentist Barry Grosser (DE00006663) for 20 years. He failed to comply with the terms and conditions of a prior order.

In July 2006 the Nursing Commission indefinitely suspended the license of Beverly C. Muhammad, a licensed practical nurse (LP00033898). She failed to comply with a prior order.

In August 2006 the Registered Counselor Program suspended the license of registered counselor Juliet M. Orpe (RC00046195) for five years. Orpe had a personal and sexual relationship with a client.

In July 2006 the Pharmacy Board revoked the certification of pharmacy technician Thomas C. Peltzer (VA00043555) for 10 years. He diverted controlled substances from his workplace for his own use.

In July 2006 the Dental Commission revoked the license of dentist Robert D. Redfield (DE00004246) for 10 years. Redfield prescribed excessive amounts of controlled substances to patients, failed to record the prescriptions in the patients» records, practiced while his license was suspended and was convicted of acquiring a controlled substance by deception.

In August 2006 the Nursing Commission indefinitely suspended the license of registered nurse Christine E. Taylor (RN00102871). She failed to comply with prior limitations on her license.

In August 2006 the Nursing Commission indefinitely suspended the license of Terrie L. Warner, a licensed practical nurse (LP00020672). She failed to comply with the terms and conditions of a prior order.

In August 2006 the Unlicensed Practice Program issued a Cease and Desist Order against Yong Yan, also known as Bill Yan. He must immediately stop the unlicensed practice of massage.

Kitsap County

In July 2006 the Pharmacy Board revoked the certification of Brendon M. Frazier, a pharmacy technician (VA00057851), for 10 years. Frazier was convicted of theft and diverted controlled substances from his workplace.

Lewis County

In August 2006 the Nursing Assistant Program indefinitely suspended the credential of nursing assistant Ilene Russell (NA00150983). She failed to comply with the terms and conditions of a prior order.

Mason County

In August 2006 the Nursing Assistant Program revoked the license of nursing assistant Larry Witt (NC10081942). His Oregon nursing assistant certificate was indefinitely suspended in April 2006 for slapping a patient while working as a nursing assistant in Oregon.

Pierce County

In August 2006 the Registered Counselor Program suspended the registration of Guy B. Ashmore, a registered counselor (RC99948981). He was convicted of driving under the influence, domestic violence, resisting arrest and reckless driving.

In August 2006 the Medical Commission entered an Agreed Order with physician David M. Brown (MD00017687). He agrees to surrender his license to practice medicine and surgery in Washington. Brown provided care that was not justified and not documented.

In August 2006 the Health Care Assistant, Nursing Assistant, Registered Counselor and X-Ray Technicians Programs revoked the credentials of Christopher L. Getz. He is credentialed as a health care assistant (HC00139814, a nursing assistant (NA00005753, a registered counselor (RC00042625) and an x-ray technician (XT00001513). Getz misrepresented his credentials and/or experience.

In August 2006 the Denturist Program suspended the license of denturist Thomas Goebel (DN00000297) for providing substandard dentures to a patient. He must pay a fine, return the fees for the dentures, and obtain education.

In July 2006 the Licensed Mental Health Counselor and Nursing Assistant Programs revoked the credentials of Christa Goggans, a licensed mental health counselor and nursing assistant (LH00003726, NA00165675), for 10 years. Goggans committed unprofessional conduct, including billing for services without seeing clients, double billing and lack of documentation to substantiate services billed.

In August 2006 the Nursing Commission revoked the credential for licensed practical nurse Steven E. Trujillo (LP0045019) for unprofessional conduct. He peeped through a window at a minor dressing for school.

San Juan County

In July 2006 the Unlicensed Practice Program issued a Cease and Desist Order against Lynne Pope. She must immediately stop practicing dental hygiene and dentistry without a license.

Spokane County

In July 2006 the Nursing Assistant Program revoked the registration of nursing assistant Brenda N. Chaffin (NA00161694) for 20 years. Chaffin forged checks and stole stocks belonging to a patient.

In July 2006 the Health Care Assistants Program indefinitely suspended the certification of health care assistant Loretta A. Larsen (HC00145031). She prescribed a drug for herself and altered her medical charts to cover it up.

Thurston County

In August 2006 the Nursing Commission indefinitely suspended the license of registered nurse Debra Marie Spencer (RN00117651). She diverted controlled substances from her workplace on several occasions.

Out of State

Canada: In August 2006 the Nursing Commission indefinitely suspended the license of Brandy M. Schneider, a licensed practical nurse licensed in Washington (LP00052649). She failed to comply with a prior order.

Georgia: In August 2006 the Nursing Assistant Program indefinitely suspended Bernard L. Black, a nursing assistant licensed in Washington (NC10080168). He failed to comply with the terms and conditions of a prior order.

Michigan: In August 2006 the Nursing Assistant Program indefinitely suspended Valerie J. Spears, a nursing assistant licensed in Washington (NA00127586). She failed to comply with the terms and conditions of a prior order.

Oregon: In July 2006 the Nursing Commission indefinitely suspended the license of Richard W. Gano, a registered nurse licensed in Washington (RN000120239). In 2005 Gano» s Oregon nursing license was revoked.

Oregon: In July 2006 the Unlicensed Practice Program issued a Cease and Desist order against Julie Grant. She must immediately stop the unlicensed practice of massage in Washington.

Oregon: In August 2006 the Nursing Commission indefinitely suspended the license of Amy M. Hardin, a registered nurse licensed in Washington (RN00134878). Hardin surrendered her Oregon nursing license after diverting narcotics. While working in Washington, she removed drugs from her employer» s supply without a physician» s order.
