State disciplines health care providers
OLYMPIA ? The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.
The department?s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency Web site. Click on "Provider Credential Search" on the left hand side of the home page (www.doh.wa.gov). The site includes information about a health care provider?s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Chelan County
In November 2006 the Nursing Assistant Program denied the application of Daniel J. Davis to practice as a nursing assistant (CA00044293). He may not reapply for five years. Davis was convicted of possessing methamphetamine.
Clark County
In October 2006 the Massage Program denied the application of Russell R. Turpin to practice as a massage practitioner (CA00008666). He may not reapply for five years. Turpin was convicted of theft.
Cowlitz County
In October 2006 the Registered Counselor Program charged registered counselor Jane M. Ashe (RC00049782) with unprofessional conduct. She allegedly failed to cooperate with the program.
In November 2006 the Nursing Assistant Program charged Kay L. Unruh, an applicant to practice as a nursing assistant (CA00045064), with unprofessional conduct. She was convicted of unlawful issuance of bank checks.
Franklin County
In November 2006 the Pharmacy Board charged pharmacist Jesus L. Alvarez (PH00020548) with unprofessional conduct. Allegations against him include failing to observe and/or correct an instructional error on a prescription given to a patient.
Grays Harbor County
In October 2006 the Pharmacy Board entered an Agreed Order with pharmacist Jack H. Jones (PH00007554). He agrees to comply with terms and conditions, including paying a fine and completing continuing education courses. Jones converted a patient?s prescription without the physician?s knowledge or permission.
In October 2006 the Pharmacy Board granted the application of Kenyon N. Lyle, Jr. to practice as a pharmacy intern (CA00056711). His registration is placed on probation and he must comply with terms and conditions. Lyle received citations for driving under the influence.
In November 2006 the Chiropractic Commission entered an Agreed Order with chiropractor Bradley A. Hensley (CH00002186). He may not take x-rays of patients until he completes continuing education. He used billing codes that reflected different areas than those he took x-rays of on patients.
King County
In October 2006 the Pharmacy Board granted the application of Clarizza Antonio to practice as a pharmacy assistant (CA00063441). Her registration is placed on probation and she must comply with terms and conditions. Antonio was convicted of theft.
In October 2006 the Dental Commission entered an Agreed Order with dentist Richard R. Garay (DE00006815). He agrees to complete continuing education, reimburse a patient and pay a fine. Garay failed to adequately document treatment of a patient.
In October 2006 the Nursing Commission modified an Agreed Order with registered nurse Melissa Kydd (RN00106835). Her license continues to be on probation and she must comply with terms and conditions.
In November 2006 the Registered Counselor Program charged Demetrius J. Morgan, an applicant to practice as a registered counselor (CA00008264), with unprofessional conduct. He has criminal convictions, including delivery of cocaine, delivery of heroin, assault and taking a motor vehicle without permission.
In November 2006 the Massage Board charged Issaac L. Rakestraw, an applicant to practice as a massage therapist (CA00008767), with unprofessional conduct. He was convicted of possessing a dangerous weapon, assault, obstructing a public servant and assault with a deadly weapon.
In November 2006 the Acupuncture Program charged acupuncturist Phavikone Sundara (AC00002249) with unprofessional conduct. He allegedly touched a patient?s breasts and kissed her on the cheek.
Mason County
In November 2006 the Dental Commission entered a Final Order with dentist Shirley O. Daniels (DE00007922). She may not perform any surgical extraction procedures until she completes continuing education courses. Daniels performed dental treatment that failed to meet the standard of care.
In November 2006 the Veterinary Board charged Jeananne Gallent, a veterinary technician (AT00000797), with unprofessional conduct. She allegedly failed to comply with prior terms and conditions placed against her registration and performed extractions without supervision.
In November 2006 the Veterinary Board charged veterinarian Gary M. Olson (VT00002961) with unprofessional conduct. He allegedly failed to comply with terms and conditions against his license and allowed a technician to perform extractions without supervision.
Pierce County
In November 2006 the Registered Counselor Program modified an order against Kimberly D. Mays, an applicant to practice as a registered counselor (CA00007786). She may not reapply until March 2007. Mays was convicted of several counts of unlawful possession of a controlled substance.
In November 2006 the Medical Commission charged physician Terrel J. Michel (MD00017273) with unprofessional conduct. Allegations against him include being terminated from Good Samaritan Hospital based on disruptive behavior which distracted from good patient care.
In November 2006 the Massage Program placed the license of massage practitioner Myong Pang (MA00018747) on probation for five years. She must comply with terms and conditions. Pang is the owner and/or operator of Classic Body Tonic Spa, Lynnwood, in which she allowed an unlicensed practitioner to perform massages and/or sexual acts on an undercover officer.
Skagit County
In November 2006 the Nursing Assistant Program charged Erika L. Ibarra, an applicant to practice as a nursing assistant (CA00046082), with unprofessional conduct. She pled guilty to assault and disorderly conduct.
Snohomish County
In November 2006 the Massage Therapy Program denied the application of Joo H. Badgley to practice as a massage therapist (CA00008073). She may not reapply for three years. Badgley practiced massage without a license and touched a client?s genitals. She was arrested and charged with prostitution.
In November 2006 the Chiropractic Commission reinstated the license of James P. Burton to practice as a chiropractor (CH00001115).
Spokane County
In November 2006 the Chiropractic Commission entered an Agreed Order with chiropractor Lyle K. Norton (CH00003331). He is reprimanded and must pay a fine and complete continuing education. Norton billed for treatments not included in a patient?s record and discouraged a patient from obtaining additional care for a suspected condition not related to chiropractic.
Thurston County
In November 2006 the Nursing Assistant Program denied the application of Ronald W. Myers to practice as a nursing assistant (CA00045027). He was arrested and charged with malicious mischief and did not cooperate with the program.
Whatcom County
In October 2006 the Nursing Commission charged Patricia S. Scheuffele, a licensed practical nurse (LP00049847), with unprofessional conduct. She allegedly diverted controlled substances from her workplace.
In October 2006 the Dental Commission entered into an Agreed Order with dentist Edwin R. Thorp (DE00004724). He agrees to comply with terms and conditions. Thorp provided services to a patient that failed to meet the standard.
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
