State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA ? The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department?s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency?s Web site. Click on "Provider Credential Search" on the left hand side of the home page (www.doh.wa.gov). The site includes information about a health care provider?s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Cowlitz County

In December 2006 the Registered Counselor Program suspended the credential of Jane M. Ashe (RC00049782), a registered counselor for at least three years. She failed to respond to an investigator.

Franklin County

In December 2006 the Pharmacy Board revoked the certification of pharmacy technician Tierney N. Becho (VA00022458) for 10 years. Becho diverted controlled substances and stole cash and merchandise from her workplace.

King County

In December 2006 the Unlicensed Practice Program issued a Cease and Desist Order against Liquin Lin. She must immediately stop the unlicensed practice of massage. Lin provided chair massage services, for a fee, to customers at Bellis Fair Mall in Bellingham.

In December 2006 the Pharmacy Board indefinitely suspended the license of pharmacist Nanette O?Shay (PH00018892). She fraudulently phoned in prescriptions for herself by pretending to be a nurse/staff member from a physician?s office. Nanette O?Shay is also known as Nanette Hollingsworth and Nanette Cotter.

In December 2006 the Nursing Assistant Program revoked the credential of nursing assistant Haley Dawn Podolske (NC10088041) for 10 years. She stole a diamond ring from a patient.

Pierce County

In December 2006 the Pharmacy Board revoked the credential of pharmacy technician Angela M. Severson (VA00045966) for 10 years. She diverted a controlled substance from her workplace and was criminally changed with unlawful possession of a controlled substance and obtaining a controlled substance by fraud, deceit or misrepresentation.

In December 2006 the Nursing Assistant Program revoked the credential of nursing assistant Evette D. Swafford (NC10047716) for 20 years. She used a deceased patient?s debit card to make unauthorized purchases. She also took a personal check belonging to a patient, forged it and cashed it. Evette D. Swafford is also known as Evette Delores Batiste.

San Juan County

In December 2006 the Pharmacy Board indefinitely suspended the license of pharmacist Claudia D. Bradley (PH00021524). She failed to comply with terms and conditions of a prior order against her license.

Snohomish County

In December 2006 the Radiologic Technologist Program suspended the certification of Lyle S. Dupree, a radiologic technologist (RT00001502), for 10 years. He sent harassing letters to former co-workers, members of the community and family members of deceased hospital patients.

Spokane County

In December 2006 the Licensed Social Worker Counselor Program revoked the license of Amy J. Angstrom (LW00007229) for five years. She failed to comply with the terms and conditions of a prior order and crossed professional boundaries with a former patient.

Thurston County

In December 2006 the Nursing Assistant Program indefinitely suspended the credential of nursing assistant Teresa M. Wood (NC10018346). She failed to comply with terms and conditions against her credential.

Out of State

Florida: In December 2006 the Medical Commission indefinitely suspended the license of Peter N. Brawn, a physician licensed in Washington (MD00014691). His credential to practice medicine in Florida was suspended after he prescribed a drug to a minor without taking a medical history, doing an examination or keeping medical records. Brawn?s Washington license was immediately suspended in October 2006.

Montana: In December 2006 the Nursing Commission revoked the license of Theresa Satiacum, a registered nurse licensed in Washington (RN00124014). Her Montana nursing license was immediately suspended in 2004 for testing positive for a controlled substance. Satiacum?s Washington license was immediately suspended in October 2006.
