State disciplines health care providers
OLYMPIA
The Washington State Department of Health has taken disciplinary actions or
withdrawn charges against health care providers in our
state.
The departments Health
Professions Quality Assurance Office works with boards, commissions, and
advisory committees to set licensing standards for more than 70 health care
professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on
the agency Web site. Click on Provider Credential Search on the left hand side
of the
Department of Health home page (www.doh.wa.gov). The site includes
information about a health care providers license status, the expiration and
renewal date of their credential, disciplinary actions and copies of legal
documents issued after July 1998. This information is also available by calling
360-236-4700. Consumers who think a health care provider acted unprofessionally
are also encouraged to call and report their complaint.

County
In July 2008 the Nursing Assistant Program charged applicant
Mandi K. Murdoch (NA00170027) with
unprofessional conduct. She was convicted of forgery and unlawful issuance of
bank checks or drafts. She allegedly failed to disclose the conviction on her
application.
Chelan
County
In July 2008 the Board of Pharmacy placed the license of pharmacist
Gary L. Hauber (PH00010118) on
probation for two years. He allowed a pharmacy technician to open and manage the
pharmacy he was responsible for. He must comply with terms and conditions set
against his license.
In July 2008 the Office of Emergency Medical Services and Trauma denied the
application of emergency medical technician applicant
Adam M. McKenney (ES01178235). He was
convicted of driving under the influence. He cannot reapply for five years.
Clark
County
In July 2008 the Chiropractic Commission charged
Troy M. Dreiling (CH00003052) with
unprofessional conduct. Allegations include failing to document any rationale or
objective need for the number of treatments he recommended for a patient,
failing to take adequate x-rays, failing to keep adequate documentation, and
giving a diagnosis without supporting documentation.
Cowlitz
County
In July 2008 the Nursing Assistant Program reprimanded
Ofelia Rodica Balea (NA00127010). She
falsified tuberculosis screening results for her employees at the adult family
home she either owned or operated.
Jefferson
County
In July 2008 the Nursing Assistant Program charged
Judy L. Hartzell (NA00133119) with
unprofessional conduct. She allegedly failed to comply with previous terms and
conditions set against her license.
King
County
In July 2008 the Midwifery Program placed
Pamela Golliet (MW00000296) on probation for 36 months. She caused risk of
harm to a mother and baby by failing to diagnose that the fetus was in breech
position. She also failed to properly respond to the mother’s elevated blood
pressure.
In July 2008 the Nursing Assistant Program placed the certification of
John B. Griffith (NC10093979) on probation for three years. He
became angry and appeared to strike a patient. He must comply with terms and
conditions set against his license.
In July 2008 the Examining Board of Psychology charged
George W. Recknagel (PY00002604) with
unprofessional conduct. He allegedly failed to comply with previous terms and
conditions set against his license.
In July 2008 the Board of Pharmacy denied the application of pharmacy assistant
applicant Moises A. Reyes
(VB00068907). He was convicted of physical control of a vehicle under the
influence. He cannot reapply for 10 years.
In July 2008 the Board of Pharmacy charged pharmacy assistant
Alexis Tobin (VB00069039) with
unprofessional conduct. She allegedly diverted drugs from the pharmacy at which
she was volunteering. Tobin also allegedly failed to respond to department
inquiries regarding the incident.
Pierce
County
In July 2008 the Nursing Assistant Program granted the reactivation application
of Katrina G. Allen (NC10087230) and placed her license on probation
for five years. She was convicted of attempted trafficking in stolen property.
She must comply with terms and conditions set against her license.
In July 2008 the Board of Pharmacy charged pharmacy technician
Misty L. Arcuri (VA00043040) with
unprofessional conduct. She was convicted of theft.
In July 2008 the Registered Counselor and Chemical Dependency Professional
Programs reinstated the credentials of
Joseph K. Myrick (RC00046581 and CP00005499).
In July 2008 the Registered Counselor Program modified the agreed order against
Leah S. Niccolocci (RC00055984). The indefinite suspension against
her credential is lifted and her registration has been placed on probation for
two years. She must comply with terms and conditions set against her
registration.
Spokane
County
In July 2008 the Dental Commission charged
Allan D. Hinkle (DE00003570) with
unprofessional conduct. Allegations include failing to keep adequate records
when administering medications, prescribing inappropriate dosages of
medications, and prescribing medications without justification.
In July 2008 the Board of Pharmacy charged pharmacy intern
Kenyon N. Lyle, Jr. (IR00066943) with
unprofessional conduct. He was convicted of driving under the influence and hit
and run attended. Lyle allegedly failed to comply with previous terms and
conditions set against his license.
In July 2008 the Board of Pharmacy charged pharmacist
Stephen Rivington (PH00041700) with
unprofessional conduct. He allegedly failed to abide by the terms of a deferred
prosecution he entered into for driving under the influence, resulting in a
bench warrant issued for his arrest. He also allegedly failed to respond to
department inquiries regarding the allegations.
In July 2008 the Board of Pharmacy placed the license of pharmacist
Michael F. White (PH00018978) on
probation for five years. He took hydrocodone from a bottle of pills not
prescribed to him and tested positive during a workplace drug screening. He must
comply with terms and conditions set against his license.
Thurston
County
In July 2008 the Nursing Commission charged licensed practical nurse
Andrea R. Wilson (LP00050785) with unprofessional conduct. She
allegedly failed to assess a patient in a timely manner.
Whatcom
County
In July 2008 the Nursing Assistant Program denied the registration application
of David L. Neal (NA00202036). He was convicted of assault – domestic
violence, theft, and assault. Neal failed to disclose the convictions on his
application. He cannot reapply for five years.
In July 2008 the Medical Commission reprimanded and fined physician
Aleixo M.D.P. Viegas (MD00041689). He
provided signed, blank prescription pads to two registered nurses to write
prescriptions for patients if he was out of the office. This is outside the
scope of practice of registered nurses.
Yakima
County
In July 2008 the Licensed Mental Health Counseling Program charged
Mark E. Heeringa (LH00008802) with
unprofessional conduct. He allegedly entered into a client referral contract
with another licensed mental health counselor and agreed to payback 50 percent
of the fees he collected from the referrals.
In July 2008 the Board of Pharmacy charged pharmacist
Lacie J. Zavala (PH00070140) with
unprofessional conduct. She allegedly filled fraudulent prescriptions for
herself and family members.
Out of State
Arizona:
In July 2008 the Massage Program reprimanded
David R. Harris (MA00022698). The
Oregon Board of Massage Therapists issued an order against him. He must comply
with terms and conditions set against his license.
Texas:
In July 2008 the Nursing Commission charged registered nurse
Matthew D. Melvyn (RN00168590) with
unprofessional conduct. He allegedly failed to comply with previous terms and
conditions set against his license.
Note to Editors:
Health care providers charged with unprofessional conduct have 20 days to
respond to the Department of Health in writing. The case then enters the
settlement process. If no disciplinary agreement can be reached, the case will
go to a hearing.
