OLYMPIA ¾ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.
The department’s Health Systems Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on
the agency Web site. Click on “Provider Credential Search” on the left hand side
of the
Department of Health home page
(www.doh.wa.gov). The site includes information about a health care provider’s
license status, the expiration and renewal date of their credential,
disciplinary actions and copies of legal documents issued after July 1998. This
information is also available by calling 360-236-4700. Consumers who think a
health care provider acted unprofessionally are also encouraged to call and
report their complaint.

In October 2008 the Massage Practitioner Program terminated the probation previously imposed against the license of M. Esther Chandler (MA00016260).
In September 2008 the Nursing Assistant Program denied the application of Trina S. Powers (NC10073765). She was convicted of theft and unlawful possession of a controlled substance – methamphetamine on more than one occasion. She failed to respond to department inquiries regarding the convictions.
In October 2008 the Board of Pharmacy charged pharmacy assistant Renee Zytniowski (VB00068155) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions set against her license.
In October 2008 the Dental Commission entered into an agreed order with dentist Terry M. Hansen (DE00004716). He failed to provide or document a pre-operative assessment and diagnosis on a patient, took a radiograph that wasn’t of diagnostic quality, and treated the patient below the standard of care when performing a root canal. He must comply with terms and conditions set against his license.
In October 2008 the Board of Pharmacy charged pharmacy technician Tara A. Jermasek (VA00044253) with unprofessional conduct. She allegedly diverted drugs from her prior place of employment for personal use.
In October 2008 the Dental Commission entered into an agreed order with dentist Syed R. Basher (DE00009509). He failed to keep adequate records of a patient’s treatment and took a radiograph that wasn’t of diagnostic quality. He must comply with terms and conditions set against his license.
In October 2008 the Naturopathy Program charged Anne L. Davis (NT00000421) with unprofessional conduct. She allegedly prescribed an excessive amount of vitamins and supplements to a patient, gave the patient expired samples, offered to sell the patient expired Oregano oil at half price, and would not reveal information regarding the oil she was selling.
In September 2008 the Nursing Assistant Program denied the application of Joyce S. Lin (NA60010437). Her license to practice as a dentist in Washington was revoked and she failed to disclose the revocation on her application.
In October 2008 the Board of Pharmacy charged pharmacy assistant Oleg Ordinartsev (VB00063030) with unprofessional conduct. He allegedly submitted false paperwork to the federal government hiding true ownership of a pharmacy and was convicted of conspiracy to commit health care fraud.
In October 2008 the Respiratory Care Practitioner Program reinstated the license of Jason M. Smith (LR00002718).
In October 2008 the Massage Practitioner Program granted the application of Angela L. Brown (MA60012520) and placed her license on probation for two years. She pleaded guilty to being under the influence of a controlled substance – methamphetamine and was convicted of negligent driving. She must comply with terms and conditions set against her license.
In October 2008 the Chemical Dependency Professional Program granted the application of Stephanie R. McKendry (CP60026487) and placed her license on probation. Her license to practice as a registered counselor was placed on probation for three years. She must comply with terms and conditions set against her license.
In October 2008 the Chemical Dependency Professional Program granted the application of Robin L. Nielsen (CP60027730) and placed her license on probation for three years. Her license to practice as a registered counselor was placed on probation. She must comply with terms and conditions set against her license.
In October 2008 the Registered Counselor Program granted the application of Joetta L. Abercrombie (RC00058349) and placed her registration on probation for three years. She was convicted of unlawful possession of a controlled substance – cocaine. She must comply with terms and conditions set against her registration.
In September 2008 the Nursing Assistant Program denied the application of Natashia M. Britt (NA00201809). She failed to respond to department inquiries regarding an arrest.
In September 2008 the Massage Practitioner Program denied the application of Sarah S. Cofrin (MA00025326). She was convicted of dangerous weapons.
In October 2008 the Medical Commission charged physician Jerome P. Rao (MD00018925) with unprofessional conduct. He allegedly shouted expletives and threw bloody irrigation at staff while in the middle of a surgery.
In October 2008 the Licensed Social Worker Program reinstated the license of Michael K. Strub (LW00004263).
In October 2008 the Naturopathy Program entered into an agreed order with Thomas J. Young (NT00000558). Young failed to properly assess and treat three patients and failed to keep adequate records. He must comply with terms and conditions against his license.
In October 2008 the Board of Pharmacy charged pharmacist Charles S. Reed (PH00010775) and his pharmacy Horen’s Drugstore (CF00003748) with unprofessional conduct. He allegedly filled and dispensed prescriptions for legend drugs. He received controlled substances from an Internet service when the prescriptions had been issued without personal interaction or patient examination by a physician. He also closed his pharmacy without notifying the department. Reed was convicted of acquiring a controlled substance by misrepresentation.
In October 2008 the Medical Commission reinstated the license of physician Alexis Hallock (MD00036935). Dr. Hallock’s license to practice is no longer restricted.
In October 2008 the Board of Pharmacy charged pharmacy assistant Jasper Townsend (VB00069427) with unprofessional conduct. He allegedly tested positive for marijuana during a urinalysis.
In September 2008 the Nursing Assistant Program denied the application of Ulamoleka T. Tuifua (NA00200521). He was convicted of two counts of vehicular assault.
In October 2008 the Medical Commission modified the order against physician David Shoemaker (MD00029733). He must comply with terms and conditions set against his license.
California: In October 2008 the Medical Commission entered into an agreed order with physician David H. Dempsey (MD00045198). Respondent provided negligent treatment to a patient in a hospital emergency department. He must comply with terms and conditions set against his license.
Colorado: In October 2008 the Nursing Commission charged registered nurse Jennifer Jarvis-Morris (RN00129770) with unprofessional conduct. She allegedly fraudulently obtained or attempted to obtain controlled substances from pharmacies by presenting forged prescriptions. Restrictions were placed against her license to practice as a registered nurse in Colorado.
Colorado: In September 2008 the Nursing Commission denied the application of registered nurse Gwendolyn Jean Scully (RN00176550). She failed to respond to department inquiries regarding previous convictions and information regarding her status with the Colorado Nurse Health Program.
Florida: In September 2008 the Nursing Commission denied the reactivation application of registered nurse Kristie Jo Berkil (RN00113716). The state of Illinois issued an order of refusal to renew on her license to practice as a registered nurse. She failed to disclose the order on her application.
