State revokes, suspends licenses, certifications, registrations of health care providers

 OLYMPIA ¾ The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Systems Quality Assurance Office works with boards, commissions and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency’s Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In June of 2009 the Chiropractic Quality Assurance Commission entered into an agreed order with Bradley L. Akridge (CHIR.CO.00001829). Akridge’s credential, which expired in 2002, will be indefinitely suspended. Akridge has been convicted of two felonies and two gross misdemeanors in Benton County.

In June of 2009 the Department of Health indefinitely suspended the credential of emergency medical technician (EMT) Brigit C. Strunk (ES01170357). Strunk fraudulently acquired prescription medication.

Franklin County

In June of 2009 the Nursing Care Quality Assurance Commission indefinitely suspended the licensed practical nursing credential of Cynthia L. Perry-Rizzo (LPN.LP.00056208) after she failed to comply with terms of an earlier order.

King County

In June of 2009 the Health Care Assistant Program entered into an agreed order with Lakisha M. Lollie (HCA.HC.00156052) under which Lollie’s credential will be indefinitely suspended. Four times, Lollie fraudulently obtained prescription medication.

In July of 2009 the Nursing Assistant Program indefinitely suspended the credential of Eilbarat Yaghoubi (NAC.NC.10096416). Yaghoubi abused a resident at a nursing home where she was employed.

Pend Oreille County

In June of 2009 the Registered Counselor and Chemical Dependency Professionals programs indefinitely suspended the credentials of Lisa K. Cyphers (RC.RC.00040713, CDP.CP.00005886). Cyphers had a romantic relationship with a client.

Pierce County

In June of 2009 the Acupuncture Program entered into an agreed order with Adam S. Jackson (ACUP.AC.00001996) in which he surrendered his license. Jackson’s license had been suspended because of sexual misconduct with a patient.

In June of 2009 the Nursing Assistant Program indefinitely suspended the credential of Tammy D. Allen (NAC.NC.10026194). Allen cashed checks she stole from a patient at a care center where she worked.

In June of 2009 the Nursing Assistant Program indefinitely suspended the credential of Melissa K. Segro (NAR.NA.00163914). Segro accepted money from a patient at a long-term care facility where she worked and failed to pay another patient for a bracelet she agreed to buy.

Spokane County

In June of 2009 the Nursing Assistant Program suspended the credential of Cassandra A. Bieber (NAC.NC.10038474) for at least three years because she was convicted twice of driving under the influence.

Stevens County

In June of 2009 the Certified Nursing Assistant Program suspended the credential of Lacey B. Sell (NAC.NC.10094293) for at least five years. She has been charged with six counts of first-degree theft and five counts of second-degree burglary. Sells took patches containing Fentanyl, a narcotic pain remedy, from a long-term care facility where she had worked.

Out of State

Arizona: In June of 2009 the Registered Counselor and Chemical Dependency Professionals programs indefinitely suspended the credentials of Elizabeth Jean Bailey-St. Claire (RC.RC.00004431, CDP.CP.00002659) for failure to comply with terms of an earlier order.

In June of 2009 the Nursing Care Quality Assurance Commission indefinitely suspended the credential of registered nurse Cathleen C. Wagoner (RN.RN.00142442) after the Arizona State Board of Nursing revoked her credential to practice in that state.

California: In June of 2009 the Nursing Care Quality Assurance Commission entered into an agreed order with registered nurse Noe Lopez (RN.RN.00173809). Lopez’s credential is indefinitely suspended. He abused a patient at a Washington hospital where he worked.

In July of 2009 the Nursing Assistant Program indefinitely suspended the credential of Shawn A. Ferreira (NAC.NC.10072710). Ferreira accepted money from residents at a Washington assisted living facility where he worked.

Oregon: In June of 2009 the Nursing Care Quality Assurance Commission indefinitely suspended the credential of registered nurse Jocelyn R. DeGroat (RN.RN.00173786) after the Oregon State Board of Nursing suspended her credential in that state.
