State disciplines health care providers

 

OLYMPIA ¾ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department’s Health Systems Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In May 2009 the Nursing Assistant Program reinstated the certification of certified nursing assistant Debi L. Begallia (NC10047876).

Clark County

 

In May 2009 the Registered Counselor Program terminated the order of probation against Glenda A. Bigalky (RC00027406).

 

Cowlitz County

 

In April 2009 the Massage Therapy Program denied the reactivation application of Tamala Rae Watkins (MA00008324). She practiced massage therapy without a valid credential.

 

Grant County

 

In May 2009 the Health Care Assistant and Nursing Assistant Programs charged certified nursing assistant Tequia F. Perkins (HC00142935, NC10065982) with unprofessional conduct. She allegedly cashed two checks from a patient totaling $3,900, which she said she was going to repay but had not; she was convicted of theft.

 

Grays Harbor County

 

In May 2009 the Board of Occupational Therapy reinstated the license of Steven R. Edwards (OT00001522).

 

King County

 

In May 2009 the Board of Hearing and Speech reprimanded speech-language pathologist Elizabeth Patricia Baker (LL60025305) and placed her license on probation for one year. She performed the duties of a speech language pathologist without a license. She must comply with terms and conditions set against her license.

 

In May 2009 the Chemical Dependency Professional Program granted the application of Bobbie Denise Catton (CP60074797) and placed her license on probation. Her registration to practice as a registered counselor was placed on probation. She must comply with terms and conditions set against her license.

 

In May 2009 the Registered Counselor Program charged Taylor Danard (RC00013399) with unprofessional conduct. She allegedly purchased a diploma from a diploma mill, misrepresented her education and training to clients and on the disclosure information she provided, identified herself as a doctor of psychology, and provided false or misleading information to department investigators.

 

In May 2009 the Massage Therapist Program granted the reactivation application of Sheri R. Lewis (MA00014271) and placed her license on probation for two years. She provided massage therapy without an active license for approximately seven years. She must comply with terms and conditions set against her license.

 

In April 2009 the Nursing Assistant Program denied the application of Cristal Yvonne Juarez Lopez (NA60061218). She was arrested for theft and obstructing a public officer, and failed to answer one of the personal data questions on her application. She didn’t respond to department inquiries regarding the arrest or missing question.

 

In April 2009 the Health Care Assistant Program denied the application of Maria Moore (HC60069492). She was convicted of theft.

 

In May 2009 the Chemical Dependency Professional Program granted the application of Rodney J. Purdy (CP60078847) and placed his license on probation. Conditions were set against his registered counselor credential. He must comply with terms and conditions set against his license.

 

In May 2009 the Dental Commission granted the application of dentist Mark Alan Stahl (DE60076890) and placed conditions against his license. He was convicted of operating a vehicle with an alcohol concentration equivalent of .15 or more. He must comply with terms and conditions set against his license.

 

In May 2009 the Nursing Assistant Program granted the application of certified nursing assistant Amy L. Ward (NC60040841) and placed her certification on probation. She entered into a deferred sentence for assault and malicious mischief. She must comply with terms and conditions set against her certification.

 

In May 2009 the Board of Psychology released Wendy J. Woodard (PY00001980) from the terms and conditions set against her license.

 

Klickitat County

 

In May 2009 the Emergency Medical Services and Trauma System Program granted the application of first responder Corey Lee Ransier (ES60084544) and placed his license on probation for three years. He was convicted of malicious mischief, trafficking in stolen property and assault more than once. He must comply with terms and conditions against his license.

 

Pierce County

 

In April 2009 the Medical Commission charged physician Jose A. Garcia (MD00011694) with unprofessional conduct. He allegedly permitted unlicensed people to administer anesthesia, perform blood draws, and inject medication into patients. He also allegedly allowed two unlicensed people to observe a patient’s surgery without the patient’s consent.

 

In May 2009 the Registered Counselor and Chemical Dependency Professional Programs charged David Charles Larsen (RC00021390, CP00000530) with unprofessional conduct. He allegedly purchased a diploma from a diploma mill, misrepresented his education and training on a job application and resume, and identified himself as a doctor of psychology.

 

In June 2009 the Nursing Assistant Program granted the application of Louise Phaedra Palazzo (NA60066299) and placed her registration on probation for one year. She was convicted of assault, malicious mischief, and domestic violence on more than one occasion. She must comply with terms and conditions set against her registration.

 

In April 2009 the Veterinary Board of Governors denied the application of veterinarian Shana M. Savikko (VT00008890). She practiced veterinary medicine without a valid credential and entered into an order for unlicensed practice with the Oregon Veterinary Medical Examining Board.

 

In May 2009 the Nursing Assistant Program charged Jordan S. Sorrick (NA00192167) with unprofessional conduct. He was convicted of assault.


 

In May 2009 the Licensed Social Worker Program charged Michael K. Strub (LW00004263) with unprofessional conduct. He allegedly purchased a diploma from a diploma mill, misrepresented his education and training to clients and on the disclosure information he provided, identified himself as a doctor of psychology, failed to maintain adequate disclosure records, and had inadequate records during an inspection by the insurance provider.

 In May 2009 the Chemical Dependency Professional Program granted the application of Dawn Michelle Winkes (CP60081200) and placed her license on probation for three years. Conditions were placed against her registration to practice as a registered counselor. She must comply with terms and conditions set against her license.

 Snohomish County

 In May 2009 the Chiropractic Commission charged Loren Ernst (CH00002486) with unprofessional conduct. He allegedly entered into a sexual relationship with a patient, employed a patient, made inappropriate comments to the employee, and suggested to staff they wear bikinis to advertise the clinic.

 

In May 2009 the Board of Psychology released Kenneth T. Nichols-Hoppe (PY00001028) from the terms and conditions against his license.

 

Spokane County

 

In June 2009 the Health Care Assistant and Nursing Assistant Programs charged certified nursing assistant Karen E. Henderson (HC00145929, NC10081756) with unprofessional conduct. She was convicted of assault, driving under the influence, and possession of a legend drug. She allegedly failed to inform the department of the convictions.

 

Whatcom County

 

In May 2009 the Registered Counselor Program reinstated the registration of Juan E. Cerrillo (RC00055523).

 

Out of State

 

Minnesota: In April 2009 the Medical Commission reinstated the license of physician Valerie L. Fox (MD00045743) and placed it on probation. She must comply with terms and conditions set against her license.

 

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
