Radioactive Materials - Laboratory Licensing The lab application is for all laboratory and manufacturing licenses who primarily use unsealed radioactive materials. If the type of package you are interested in is not available on-line, please contact us to receive a printed copy. How to Determine your Application Fee for New Licensees Please find your fee category and the corresponding annual fee that most nearly matches your proposed operation from the Current Annual Fees. It is highly recommended that you contact the appropriate Licensing Manager to avoid mistakes that could delay your licensing request. The total fee you must submit is the current annual license fee, plus the new application license fee of $200.
