How to Determine your Application Fee for New Licensees

Please find your fee category and the corresponding annual fee that most nearly matches your proposed operation from the Current Annual Fees.  It is highly recommended that you contact the appropriate Licensing Manager to avoid mistakes that could delay your licensing request. The total fee you must submit is the current annual license fee, plus the new application license fee of $200.
